The Value of Combined Assessment Using CURB-65 Score,Procalcitonin,and Glycated Hemoglobin in Predicting Prognosis of Severe Pneumonia in Diabetic Patients with Community-Acquired Pneumonia

Abstract:Objective To analyze the efficacy of CURB-65 score,procalcitonin and glycosylated hemoglo-bin in evaluating short-term prognosis of emergency diabetes patients with community acquired severe pneumo-nia.Methods From January 2023 to June 2024,100 patients with emergency diabetes complicated with com-munity acquired severe pneumonia who were admitted to the community health service center in Donghu Street,Fengze District,Quanzhou City,Fujian Province were selected as the study subjects.According to the patient's outcome 30 days after admission,they were divided into a survival group(64 cases)and a death group(36 cases).Compare the gender,age,smoking history,length of hospital stay,CURB-65 score,disease history(cardiovascular disease,cerebrovascular disease,respiratory system disease),physical signs(systolic blood pressure,respiratory rate,heart rate),laboratory test results(procalcitonin,white blood cell count,C-reactive protein,D-dimer,glycated hemoglobin,urea nitrogen,blood creatinine,blood albumin)between two groups of patients.Results Compared with the survival group,the death group had older age,higher CURB-65 scores,and a higher proportion of patients with cerebrovascular and respiratory diseas-es(P<0.001);The proportion of systolic blood pres-sure<90mmHg is higher(P<0.001),and laboratory tests show significantly higher levels of procalcitonin,C-reactive protein,D-dimer,glycated hemoglobin,urea nitrogen,blood creatinine,and blood albumin(P<0.05).Multivariate logistic regression analysis found that high CURB-65 scores,high levels of calcitonin,and high levels of hyperglycemic hemoglobin were independent risk factors for mortality in patients 30 days after admission(P<0.05).The ROC curve analysis showed that the AUC value of the combination of reduced CURB-65 score,procalcitonin and glycosylated hemoglobin in evaluating the prognosis of patients with diabetes complicated with community acquired severe pneumonia was 0.913,the sensitivity was 93.49%,and the specificity was 91.50%,which were significantly higher than the evaluation efficacy of a single index.Conclusion CURB-65 score,procalcitonin and glycosylated hemoglobin are independent risk factors for the death of emergency diabetes patients with community-acquired severe pneumonia 30 days after admission,and the combination of the three has a high value in evaluating the prognosis of patients.
