Clinical Study on the Use of Single-dose Insulin Bolus for the Treatment of Diabetic Ketoacidosis

Abstract:Objective To analyze the clinical value of a single-dose insulin bolus therapy for patients with diabetic ketoacidosis(DKA).Methods A total of 100 patients with diabetic ketoacidosis who visited Linqu County People's Hospital from January 2019 to January 2024 were enrolled in this study.The patients were randomly divid-ed into two groups using a random number table:the observation group(treated with single-dose insulin bolus ther-apy,50 cases)and the control group(treated with low-dose continuous infusion therapy,50 cases).The clinical ef-ficacy,recovery time of clinical parameters,acidosis markers,blood glucose levels,myocardial enzyme profile,and the occurrence of adverse events were compared between the two groups.Results There was no significant differ-ence between the two groups in terms of overall clinical efficacy and the total incidence of adverse events(P>0.05).Compared with the control group,the observation group showed significantly shorter times to achieve target blood glucose,negative urinary ketones,and normalization of blood pH.Before treatment,there were no significant differ-ences between the two groups in acidosis markers(β-hydroxybutyrate,blood lactate,blood ketone bodies),blood glucose parameters(fasting blood glucose,2-hour postprandial blood glucose,glycated hemoglobin),or myocardi-al enzyme profile markers(creatine kinase,creatine kinase isoenzyme,aspartate aminotransferase)(P>0.05).After treatment,both groups showed significant reductions in acidosis markers,blood glucose levels,and myocardial enzyme markers(P<0.001).Additionally,compared with the control group,the observation group had significantly lower levels of β-hydroxybutyrate,blood lactate,blood ketone bodies,fasting blood glucose,2-hour postprandial blood glucose,glycated hemoglobin,creatine kinase,creatine kinase isoenzyme,and aspartate aminotrans-ferase.Conclusion For emergency patients with diabetic ketoacidosis,the single-dose insulin bolus therapy demonstrates significant clinical efficacy.It shortens the recovery time for clinical parameters,effectively improves acidosis,blood glucose levels,and myocardial enzyme profile markers,with a low risk of adverse events.
