Analysis of Antimicrobial Use in Acute Exacerbation of Chronic Obstructive Pulmonary Disease at Youxi County Traditional Chinese Medicine Hospital

Abstract:Objective To investigate the characteristics of antimicrobial drug use in hospitalized patients with acute exacerbations of chronic obstructive pulmonary disease (AECOPD) at Youxi County Hospital,Fujian Province,from 2022 to 2023,and provide theoretical guidance for rational drug use in clinical practice. Methods A retro-spective analysis was conducted on the use of antimicrobial agents in 232 patients with AECOPD as the primary dis-charge diagnosis at our hospital from 2022 to 2023. Results A total of 13 antimicrobial agents from 5 drug classes were used in 272 instances. The most frequently used drugs were β-lactams,accounting for 243 instances (89.34%),followed by quinolones,which accounted for 24 instances (8.82%). Among the 232 patients who received antimicro-bial treatment,monotherapy was most common,used in 203 cases (87.50%). The average duration of antimicrobial therapy was (8.04±2.48) days. The duration of β-lactam use was significantly longer than that of other drug classes (P<0.05). The top three drugs in terms of defined daily doses (DDDs) were all β-lactams. The drug utilization index (DUI) for all five classes of antibiotics surveyed was<1. In terms of combination therapy,28 patients (12.07%) received two-drug regimens,while only 1 patient (0.43%) received a three-drug regimen. Additionally,the results of drug sen-sitivity testing revealed a significant issue of varying degrees of resistance to commonly used antibiotics among sever-al major Gram-negative and Gram-positive bacteria. Conclusion In our hospital,β-lactams are the most commonly used antimicrobial agents for AECOPD. Combination therapy is generally appropriate,but improvements are needed in evaluating combination therapies,individual-izing monotherapy,monitoring resistance,and involving pharmacists in intervention to optimize drug regimens.
