Analysis of Risk Factors for Catheter-Related Bloodstream Infection in Neonates and Construction of A Nomogram Prediction Model

Abstract:Objective To explore the risk factors of catheter-related bloodstream infections(CRBSI)in neonates and predict the risk of CRBSI associated with central venous catheterization.Methods Clinical records of 386 neonates who underwent central venous catheterization at Tangshan Maternal and Child Health Hospital from January 2020 to December 2024 were collected,and the neonates were divided into the CRBSI group(37 cases)and the non-CRBSI group(349 cases)based on whether CRBSI occurred.The t-test and χ2 test were used to compare clinical data between the two groups.Multivariate logistic stepwise regression analyzed influencing factors for neonatal CRBSI,and a nomogram prediction model was constructed.Results Among 386 neonates with central venous catheters,37 developed CRBSI(9.59%),with 42 pathogenic strains isolated(predominantly gram-positive bacteria,57.14%).Risk factors for neonatal CRBSI included:birth weight<1.5 kg(OR=0.523,95%CI:0.381-0.716),low 5-minute Apgar score(OR=0.393,95%CI:0.263-0.586),≥2 puncture attempts(OR=2.401,95%CI:1.629-3.540),catheter indentation time≥2 weeks(OR=2.832,95%CI:1.865-4.299),and elevated procalcitonin levels(OR=2.479,95%CI:1.669-3.684).The nomogram model showed an AUC of 0.896(95%CI:0.844-0.948)for predicting CRBSI,with 67.53%specificity and 92.04%sensitivity.Conclusion The nomogram model based on risk factors for neonatal CRBSI demonstrates good predictive performance and can serve as an important tool for guiding clinical treatment.
