Effect of Vitrectomy Combined with Internal Limiting Membrane Peeling on Visual Acuity and Macular Edema in Patients with Refractory Diabetic Macular Edema

Abstract：Objective To explore the effect of vitrectomy(PPV)combined with internal limiting membrane peeling(ILMP)on visual acuity and macular edema in patients with refractory diabetic macular edema(DME).Methods A retrospective study was conducted on 185 patients with refractory DME admitted to Meishan People's Hospital from January 2018 to January 2025.Based on the treatment methods in the data,they were divided into a control group(n=91)and a combined group(n=94).The control group received PPV alone,and the combined group received PPV combined with ILMP.Best corrected visual acuity,central macular thickness(CMT),severity of macular edema at 6 months postoperatively,and adverse reactions were compared between the two groups.Results The logMAR values and CMT in both groups at 1,3,and 6 months after surgery were significantly lower than those before surgery(P<0.05),and they showed a continuous downward trend over time(P<0.05).The logMAR values of the combined group at 1,3,and 6 months after surgery were significantly lower than those of the control group at the same time points(P<0.05).The distribution of macular edema severity grades in the combined group was significantly better than that in the control group(P<0.05),with milder edema in the combined group.Within 6 months after surgery,there was no statistically significant difference in the total incidence of adverse reactions between the combined group and the control group(6.38%vs 9.89%,P>0.05).Conclusion PPV combined with ILMP is more effective than PPV alone in treating refractory DME.It can not only effectively improve visual function but also reduce the degree of macular edema,with a favorable safety profile.
