The Value of Serum Uric Acid/Creatinine Ratio in Evaluating Early Renal Injury in Children with Henoch-Schönlein Purpura

Abstract:Objective To explore the value of serum uric acid to creatinine ratio(SUA/Cr)in evaluating early renal injury in children with Henoch-Schönlein purpura(HSP).Methods A total of 180 children with HSP admitted to Xichang City People's Hospital from January 2021 to January 2025 were retrospectively selected.According to urinalysis results,they were divided into a renal injury group(n=53)and a non-injury group(n=127).Serum uric acid(SUA),creatinine(Cr),and SUA/Cr were compared between the two groups.The predictive value of SUA/Cr for early renal injury in children with HSP was analyzed using ROC curves.General clinical data of the two groups were compared,and multivariate logistic stepwise regression was used to analyze influencing factors for early renal injury in children with HSP.Results SUA,Cr,and SUA/Cr in the injury group were significantly higher than those in the non-injury group[(394.62±31.24)μmol/L vs(329.45±30.51)μmol/L,P<0.001;(72.87±6.52)μmol/L vs(67.55±6.32)μmol/L,P<0.001;5.50±0.73 vs 4.88±0.68,P<0.001].ROC curve analysis showed that the AUC of SUA/Cr for predicting early renal injury in children with HSP was 0.910,which was significantly better than that of SUA alone(0.853)or Cr alone(0.774)(Z=8.547,9.254,P<0.001).The injury group had higher frequencies of recurrent rash,gastrointestinal symptoms,platelet count,D-dimer,fibrinogen,IgA,and urinary microalbumin levels than the non-injury group(P<0.05).Multivariate logistic stepwise regression analysis showed that recurrent rash(OR=1.984,95%CI:1.182-3.328),elevated D-dimer(OR=2.061,95%CI:1.267-3.350),elevated IgA(OR=2.106,95%CI:1.275-3.479),and SUA/Cr≥5.19(OR=3.010,95%CI:1.510-6.001)were independent risk factors for early renal injury in children with HSP(P<0.05).Conclusion Elevated SUA/Cr is significantly associated with early renal injury in children with HSP and shows good predictive performance for early renal injury.
