Analysis of Influencing Factors for Frailty and Development of A Nomogram Model in Patients with Hematological Malignancies Undergoing Chemotherapy

Abstract:Objective To explore the influencing factors of frailty in patients with hematological malignancies undergoing chemotherapy and construct a nomogram model.Methods A total of 326 patients with hematological malignancies undergoing chemotherapy in the First Affiliated Hospital of Air Force Medical University from January 2023 to August 2025 were selected.The patients were divided into a non-frailty group(n=192)and a frailty group(n=134)using the Fried frailty phenotype scale.General data,laboratory indicators(albumin,hemoglobin),and relevant scale scores(activities of daily living,anxiety and depression,nutritional risk,sleep status)were compared between the two groups.Multivariate logistic stepwise regression analysis was used to explore the influencing factors of frailty in these patients.A nomogram was drawn,and the predictive value of the nomogram model was evaluated using the receiver operating characteristic curve.Results The age of patients in the frailty group was significantly higher than that in the non-frailty group,while hemoglobin and albumin levels were significantly lower.The proportions of patients with a monthly income<3 000 yuan,smoking history,≥4 chemotherapy sessions,exercise frequency<3 times per week,impaired activities of daily living,anxiety and depression,sleep disorders,and nutritional risk were all significantly higher in the frailty group than in the non-frailty group,with statistically significant differences(all P<0.05).Multivariate regression analysis showed that older age(OR=1.831,95%CI:1.264-2.652),anxiety and depression(OR=2.277,95%CI:1.434-3.617),and exercise frequency<3 times per week(OR=2.321,95%CI:1.419-3.796)were independent risk factors for frailty in patients with hematological malignancies undergoing chemotherapy,while higher hemoglobin(OR=0.894,95%CI:0.833-0.959)and higher albumin levels(OR=0.807,95%CI:0.715-0.912)were independent protective factors(P<0.05).The nomogram model showed a consistency index of 0.886(95%CI:0.836-0.935).The area under the curve(95%CI)of the nomogram model for predicting frailty was 0.917(0.866-0.965),with a specificity of 0.776 and a sensitivity of 0.881.Conclusion Age,anxiety and depression,exercise frequency,hemoglobin,and albumin are influencing factors of frailty in patients with hematological malignancies undergoing chemotherapy.The nomogram model constructed based on these factors shows good predictive performance.
