Malnutrition Status and Its Correlation with Frailty and Quality of Life in Elderly Patients with Hip Fracture

Abstract:Objective To understand the nutritional status of malnutrition in elderly patients with hip fractures and to analyze its correlation with frailty status and quality of life.Methods A cross-sectional study was conducted.A total of 300 elderly inpatients with hip fractures admitted to the Orthopedics Department of Xiamen Hongai Hospital from February 2022 to June 2025 were selected by convenience sampling.According to the Mini Nutritional Assessment-Short Form(MNA-SF)scores,patients were divided into a malnutrition group(236 cases)and a non-malnutrition group(64 cases).Baseline data questionnaire,MNA-SF,FRAIL Scale,and WHO Quality of Life-BREF(QOL-BREF)Scale were used to investigate nutritional status,frailty status,and quality of life.Point-biserial correlation analysis was used to analyze the correlation between nutritional status and frailty status as well as quality of life.Binary logistic regression analysis was used to analyze the influencing factors of malnutrition in elderly patients with hip fractures.Results In the malnutrition group,the proportion of patients aged≥65 years,the proportion with≥2 comorbidities,body mass index(BMI),and FRAIL scores were higher than those in the non-malnutrition group,while QOL-BREF scores were lower than those in the non-malnutrition group,with statistically significant differences(P<0.05).Point-biserial correlation analysis showed that malnutrition was positively correlated with FRAIL scores(r=0.567,P<0.001)and negatively correlated with QOL-BREF scores(r=-0.538,P<0.001).Multivariate logistic regression analysis showed that comorbidities,FRAIL scores,and BMI were independent risk factors for malnutrition in elderly patients with hip fractures,while QOL-BREF scores were an independent protective factor(OR=1.754,95%CI:1.044-2.949,P=0.035;OR=1.872,95%CI:1.163-3.014,P=0.010;OR=1.826,95%CI:1.071-3.112,P=0.027;OR=0.510,95%CI:0.311-0.837,P=0.008).Conclusion The detection rates of malnutrition and malnutrition risk are high in elderly patients with hip fractures,which leads to frailty and affects patients'quality of life.Medical staff should identify patients with malnutrition as early as possible,intervene timely,and improve patient prognosis.
