Research Progress on Relationship Between Osteonecrosis of the Femoral Head and Pyroptosis

Abstract:Osteocyte death in osteonecrosis of the femoral head(ONFH)is a key link leading to the collapse of the femoral head,and its mechanism involves a variety of cell death modes.Recent studies have suggested that pyroptosis,as a pro-inflammatory programmed cell death mediated by Gasdermin D(GSDMD),plays an important role in the pathogenesis of ONFH.Under the influence of pathogenic factors such as hormones or alcohol,the NLRP3 inflammasome/caspase-1 signaling pathway is activated,cleaving GSDMD to form cell membrane pores,leading to osteocyte lysis and the release of a large number of inflammatory factors such as IL-1β,thus triggering a vicious cycle of osteoclast activation,osteogenic inhibition and microcirculation disorder,and jointly promoting disease progression.Currently,studies mostly focus on confirming the existence of this pathway,but the understanding of the initiation specificity of pyroptosis under different causes and its interaction with other death modes remains limited.Future studies should integrate single-cell sequencing and gene editing techniques to deeply analyze the precise regulatory network of pyroptosis in ONFH,and provide a new perspective for the development of early intervention strategies targeting key pyroptosis molecules.
