Effect of Perioperative Esketamine on Prevention of Postpartum Depression following Cesarean Delivery:A Meta-Analysis

Abstract:Objective To systematically evaluate the efficacy of perioperative esketamine in preventing postpartum depression(PPD)in parturients undergoing cesarean delivery.Methods We searched PubMed,Embase,the Cochrane Library,Web of Science,CBM,Wanfang,CNKI,and VIP databases from inception to February 2025.Randomized controlled trials(RCT)investigating the effect of perioperative esketamine on PPD in parturients undergoing cesarean delivery were included.Two researchers independently screened studies,extracted data,and assessed the risk of bias.Meta-analysis was performed using RevMan 5.4 software.Results Eleven RCTs involving 2258 patients(esketamine group:n=1 117;control group:n=1 141)were included.The results showed that the incidence of short-term(≤7 days)PPD(RR=0.54;95%CI:0.44-0.66,P<0.05)and longer-term(≥28 days)PPD(RR=0.64,95%CI:0.45-0.92,P<0.05)in the esketamine group was significantly lower than that in the control group;however,the incidence of intraoperative hallucinations(RR=16.66,95%CI:4.03-68.89,P<0.05)was significantly higher in the esketamine group.Regarding postoperative analgesia,patients in the esketamine group had significantly lower VAS scores at 4 h,12 h,24 h,and 48 h postoperatively compared to the control group(P<0.05).Conclusion Perioperative application of esketamine can reduce the incidence of PPD after cesarean delivery and provide effective postoperative analgesia,but it carries an increased risk of intraoperative adverse reactions.
