Effects of Conbercept Combined with Laser Therapy on Retinal Microstructure and Influencing Factors of Therapeutic Response in Diabetic Macular Edema

Abstract:Objective To explore the effects of conbercept combined with laser therapy on retinal microstructure and the influencing factors of therapeutic response in diabetic macular edema(DME).Methods A total of 108 patients with DME admitted to the Department of Ophthalmology,Dongguan People's Hospital from April 2019 to April 2021 were selected and randomly divided into the control group(n=54)and the observation group(n=54)using the random number table method.The control group was treated with laser therapy alone,while the observation group was treated with intravitreal injection of conbercept combined with laser photocoagulation.Changes in retinal microstructure and visual function indicators before and after treatment were compared between the two groups,including central retinal thickness(CRT),average macular volume(AMV),integrity of the ellipsoid zone(EZ),best corrected visual acuity(BCVA),and adverse reactions.Multivariate logistic regression analysis was used to analyze the influencing factors of the effectiveness of intravitreal injection of conbercept combined with laser therapy in patients with DME.Results After treatment,the CRT and AMV of both groups were lower than those before treatment,while the EZ integrity was higher than that before treatment;additionally,the observation group showed greater improvement than the control group,with statistically significant differences(P<0.05).After treatment,the BCVA of both groups was higher than that before treatment,and the BCVA of the observation group was higher than that of the control group,with a statistically significant difference(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).The duration of diabetes,duration of DME,peripheral blood neutrophil,glycated hemoglobin level,and fasting blood glucose level in the responsive group were lower than those in the non-responsive group,with statistically significant differences(P<0.05).Multivariate logistic stepwise regression analysis showed that the duration of diabetes,peripheral blood neutrophil,and glycated hemoglobin were independent risk factors affecting the effectiveness of intravitreal conbercept injection combined with laser therapy in patients with DME(OR=2.330,95%CI:1.124-4.831,P=0.023;OR=3.013,95%CI:1.384-6.561,P=0.005;OR=3.228,95%CI:1.671-6.237,P<0.001).Conclusion Conbercept combined with laser therapy can effectively improve the retinal microstructure and visual function of DME patients,and its efficacy is independently affected by multiple factors such as the duration of diabetes,glycated hemoglobin,and peripheral blood neutrophil.
