Effect of Intra-Aortic Balloon Pump Combined with Drug Therapy on Cardiac Pumping Function and Short-Term Clinical Outcomes in Patients with ST-Segment Elevation Myocardial Infarction Complicated by Heart Failure

Abstract:Objective To investigate the effect of intra-aortic balloon pump(IABP)combined with drug therapy on cardiac pump function and short-term clinical outcomes in patients with ST-segment elevation myocardial infarction(STEMI)complicated by heart failure.Methods A total of 94 STEMI patients who developed heart failure after successful emergency percutaneous coronary intervention at Beijing Daxing District People's Hospital from January 2020 to January 2025 were prospectively selected and randomly divided into an observation group(n=47)and a control group(n=47)using a random number table.The control group received standard drug therapy,while the observation group received additional IABP therapy.Cardiac pumping function[left ventricular ejection fraction(LVEF),stroke volume(SV),cardiac output(CO),cardiac index(CI)],hemodynamic parameters[pulmonary capillary wedge pressure(PCWP),heart rate(HR),mean arterial pressure(MAP)],myocardial enzyme profiles[cardiac troponin I(cTnI),lactate dehydrogenase(LDH),creatine kinase isoenzyme MB(CK-MB)],short-term clinical outcomes,adverse reactions,and IABP-related complications were compared between the two groups.Results After treatment,LVEF,SV,CO,CI,and MAP in both groups were higher than before treatment(P<0.05),and these parameters were higher in the observation group than in the control group(P<0.05).PCWP,HR,cTnI,CK-MB,and LDH in both groups were lower than before treatment(P<0.05),and these parameters were lower in the observation group(P<0.05).After 30 days of follow-up,the incidence of major adverse cardiovascular events(MACE)in the observation group(4.26%)was lower than that in the control group(17.02%)(P<0.05).There was no significant difference in the overall incidence of adverse reactions and IABP-related complications between the two groups(P>0.05).Conclusion IABP combined with drug therapy can significantly improve cardiac pumping function and hemodynamics,reduce myocardial enzyme levels,decrease the incidence of MACE,and improve short-term clinical outcomes in patients with STEMI complicated by heart failure.
