The Association between Different OCT Stages and Changes in BCVA and CMT following Vitrectomy with Epiretinal and Internal Limiting Membrane Peeling in Patients with Idiopathic Epiretinal Membrane

Abstract:Objective To analyze the effects of vitrectomy combined with epiretinal membrane peeling and internal limiting membrane peeling on best-corrected visual acuity(BCVA)and central macular thickness(CMT)in patients with idiopathic epiretinal membrane(IERM)at different stages classified by Optical Coherence Tomography(OCT),both before and after surgery.Methods A total of 90 patients with idiopathic epiretinal membrane(IERM)admitted to Tangshan Eye Hospital Affiliated to North China University of Science and Technology from July 2021 to July 2023 were selected.According to the different stages determined by OCT,they were divided into StageⅠGroup(30 cases),StageⅡGroup(30 cases),and StageⅢGroup(30 cases).The BCVA,CMT,subfoveal choroidal thickness(SFCT),choroidal thickness at 1 000 μm nasal to the fovea(NFCT),choroidal thickness at 1 000 μm temporal to the fovea(TFCT),and mean sensitivity(MS)of the three groups were compared before surgery,1 month after surgery,and 3 months after surgery.Results Before surgery,1 month after surgery,and 3 months after surgery,there were statistically significant differences in BCVA and MS among the three groups(P<0.001).At 1 month and 3 months after surgery,the MS of the Stage Ⅱ Group and Stage Ⅲ Group was higher than that before surgery(P<0.05);at 3 months after surgery,the BCVA of the StageⅡ Group and Stage Ⅲ Group was higher than that before surgery(P<0.05);at 3 months after surgery,the BCVA and MS of the Stage II Group and Stage III Group were higher than those at 1 month after surgery(P<0.05).When comparing BCVA and MS among the groups at different time points,the StageⅡ Group showed the most significant difference(P<0.05).Before surgery,1 month after surgery,and 3 months after surgery,there were statistically significant differences in CMT,SFCT,NFCT,and TFCT among the three groups(P<0.001).At 1 month and 3 months after surgery,the CMT,SFCT,NFCT,and TFCT of the three groups were lower than those before surgery(P<0.05);at 3 months after surgery,the CMT,SFCT,NFCT,and TFCT of the three groups were lower than those at 1 month after surgery(P<0.05).When comparing CMT,SFCT,NFCT,and TFCT among the groups at different time points,the StageⅠ Group showed the most significant difference(P<0.05).Conclusion In IERM patients,a higher OCT stage is associated with lower BCVA and greater CMT.Vitrectomy combined with epiretinal membrane peeling and internal limiting membrane peeling is an effective treatment for IERM.Earlier disease stage is associated with a greater magnitude and stability of visual improvement and more ideal structural recovery.
