Influencing Factors for Lower Extremity Deep Venous Thrombosis in Burn Patients and Construction of a Nomogram Prediction Model

Abstract:Objective To explore the influencing factors of lower extremity deep venous thrombosis(DVT)in burn patients,and to construct and validate a nomogram prediction model.Methods A total of 208 patients who were treated in the Department of Burn Surgery,the First Affiliated Hospital of Air Force Medical University from March 2022 to February 2025 were retrospectively selected.According to the occurrence of DVT during hospitalization,the patients were divided into the DVT group(31 cases)and the non-DVT group(177 cases).The general clinical data of the two groups were compared.Multivariate logistic regression analysis was used to analyze the influencing factors of DVT in burn patients.A nomogram prediction model was constructed based on R software,and the receiver operating characteristic(ROC)curve was used for validation.Results Age,body mass index,prevalence of diabetes mellitus,proportion of patients with total burn surface area(TBSA)≥30%,proportion of patients with lower extremity burns,proportion of patients with bed rest time≥30 days,proportion of patients with deep vein catheterization,prevalence of wound infection/sepsis,usage rate of low-molecular-weight heparin,D-dimer level,fibrinogen level,and tumor necrosis factor-α level in the DVT group were higher than those in the non-DVT group(P<0.05).Multivariate logistic regression analysis showed that TBSA≥30%(OR=2.241,95%CI:1.297-3.872),lower extremity burns(OR=1.608,95%CI:1.184-2.183),bed rest time≥30 days(OR=2.472,95%CI:1.056-5.787),deep vein catheterization(OR=2.514,95%CI:1.405-4.500),wound infection/sepsis(OR=4.315,95%CI:1.457-12.799),and elevated D-dimer level(OR=1.627,95%CI:1.230-2.154)were independent risk factors for DVT in burn patients(P<0.05).ROC curve analysis showed that the area under the curve of the nomogram prediction model for predicting DVT occurrence in burn patients was 0.885,with a sensitivity and specificity of 80.65%and 89.83%,respectively.Conclusion TBSA≥30%,lower extremity burns,bed rest time≥30 days,deep vein catheterization,wound infection/sepsis,and elevated D-dimer level are independent risk factors for DVT in burn patients.The nomogram model constructed based on the above factors can individually predict the probability of DVT in burn patients and provide a quantitative basis for the diagnosis and treatment of DVT.
