Impact of Intraoperative Physiological Parameters on Postoperative Length of Stay in Patients Undergoing Unilateral Bicompartmental Knee Arthroplasty

Abstract:Objective To analyze the impact of intraoperative physiological monitoring parameters on the postoperative length of stay(LOS)in patients undergoing unilateral bicompartmental knee arthroplasty(BKA),and to provide a basis for optimizing perioperative management.Methods A total of 221 patients with knee osteoarthritis(KOA)who underwent unilateral BKA at Bozhou People's Hospital from March 2024 to February 2025 were retrospectively selected.Clinical characteristics,including medical records,surgical records,and intraoperative anesthesia records,were extracted from the hospital's electronic medical record system.LOS was used as the primary outcome measure.Univariate analysis,correlation analysis,and multiple stepwise linear regression were employed to analyze the relationship between intraoperative physiological monitoring parameters and postoperative LOS.Results This study included 221 BKA patients,all of whom completed measurements of 25 basic indicators.Among them,86 patients underwent additional testing of intraoperative fluid intake,13 blood and blood gas analysis indicators,totaling 39 indicators.Age was positively correlated with postoperative LOS(P<0.05).In the 221 patients,the fluctuation range of arterial systolic blood pressure was significantly negatively correlated with postoperative LOS(r=-0.157,P=0.020).In the 86 patients with blood biochemistry and arterial blood gas data,arterial partial pressure of carbon dioxide(PCO2)was significantly positively correlated with postoperative LOS(r=0.488,P<0.001).Multiple stepwise linear regression analysis showed that ionized calcium(Ca2+),PCO2,bicarbonate(HCO3-),and blood pH were independent influencing factors for LOS(all P<0.05).The multiple stepwise linear regression model for factors influencing postoperative LOS in unilateral BKA patients demonstrated good overall fit(R2=0.425,adjusted R2=0.396),explaining 39.6%of the variance in LOS.Conclusion Intraoperative Ca2+,PCO2,HCO3-,and blood pH are closely associated with postoperative LOS in unilateral BKA patients and have potential value in predicting LOS.
