Relationship Between the Age-Adjusted Charlson Comorbidity Index and Prognosis in Patients with Advanced Liver Cancer

Abstract:Objective To analyze the relationship between the age-adjusted Charlson comorbidity index(ACCI)and the prognosis of patients with advanced liver cancer receiving chemotherapy combined with immunotherapy.Methods A total of 156 patients with advanced liver cancer admitted to Hospital 363 from January 2022 to January 2024 were retrospectively selected.All patients received chemotherapy combined with immunotherapy.According to the ACCI score,they were divided into a low-score group(ACCI score<9,61 cases)and a high-score group(ACCI score≥9,95 cases).Kaplan-Meier survival analysis was used to compare the overall survival(OS)between the two groups.Based on the prognosis within one year of follow-up,patients were divided into a survival group(79 cases)and a death group(77 cases).General data,ACCI score,liver function grade,and pathological characteristics of liver cancer were compared between the two groups.Collinearity diagnostics and multivariate Cox regression analysis were performed to identify factors influencing the prognosis of advanced liver cancer patients treated with chemotherapy combined with immunotherapy.Results The OS of the low-score group was significantly higher than that of the high-score group(P<0.05).Compared with the survival group,the death group had higher performance status(PS)score,ACCI score,alpha-fetoprotein(AFP)level,and higher proportions of liver cirrhosis and portal vein tumor thrombus(P<0.05).Collinearity diagnostics and multivariate Cox regression analysis showed that PS score(HR=2.351,95%CI:1.198-4.616),ACCI score(HR=1.284,95%CI:1.126-1.463),AFP level(HR=1.011,95%CI:1.007-1.015),portal vein tumor thrombus(HR=2.574,95%CI:1.611-4.114),and liver cirrhosis(HR=1.974,95%CI:1.200-3.248)were independent risk factors for the prognosis of advanced liver cancer patients treated with chemotherapy combined with immunotherapy(P<0.05).Conclusion The ACCI score can predict the prognosis of patients with advanced liver cancer after chemotherapy combined with immunotherapy.PS score,ACCI score,AFP level,liver cirrhosis,and portal vein tumor thrombus are independent risk factors for the prognosis of these patients.
