Relationship Between Serum NLRP3 and Caspase-1 Levels and the Degree of Intracranial Atherosclerotic Stenosis in Patients with Cerebral Infarction

Abstract:Objective To explore the relationship between serum levels of nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)and Caspase-1 and the degree of atherosclerotic stenosis of intracranial arteries(middle cerebral artery,anterior cerebral artery,and posterior cerebral artery)in patients with cerebral infarction.Methods Patients with cerebral infarction accompanied by intracranial atherosclerotic stenosis who were treated at China RongTong Medical Healthcare Group Co.,Ltd.Handan 285 Hospital from January 2021 to January 2025 were retrospectively selected and included in the observation group(103 cases).Patients with cerebral infarction without intracranial atherosclerotic stenosis who visited China RongTong Medical Healthcare Group Co.,Ltd.Handan 285 Hospital during the same period were included in the control group(107 cases).The levels of NLRP3 and Caspase-1 were compared between the two groups.Based on computed tomography angiography results,the observation group was further divided into a mild-to-moderate group(64 cases)and a severe group(39 cases).Differences in serum triacylglycerol,NLRP3,and Caspase-1 levels were compared between these two groups.Multivariate Logistic regression analysis was used to identify factors influencing the degree of intracranial atherosclerotic stenosis in patients with cerebral infarction.The value of NLRP3 and Caspase-1 in predicting the degree of intracranial atherosclerotic stenosis was evaluated using ROC curve analysis.Results Serum levels of NLRP3 and Caspase-1 in the observation group were significantly higher than those in the control group(P<0.001).In the severe group,levels of triacylglycerol,total cholesterol,NLRP3,and Caspase-1 were significantly higher than those in the mild-to-moderate group,while levels of alanine aminotransferase and homocysteine were significantly lower(P<0.001).Multivariate Logistic regression analysis showed that triacylglycerol,total cholesterol,alanine aminotransferase,homocysteine,NLRP3,and Caspase-1 were influencing factors for the degree of intracranial atherosclerotic stenosis in patients with cerebral infarction(OR=1.551,95%CI:1.077-2.233,P=0.019;OR=4.745,95%CI:1.025-21.971,P=0.047;OR=4.655,95%CI:1.122-19.317,P=0.035;OR=3.511,95%CI:1.240-9.942,P=0.018;OR=8.542,95%CI:1.596-45.730,P=0.013;OR=12.579,95%CI:1.861-85.030,P=0.010).ROC curve analysis revealed that the AUC for the combined prediction of NLRP3 and Caspase-1 for the degree of intracranial atherosclerotic stenosis was 0.900,which was better than the individual predictions of NLRP3(AUC=0.817)and Caspase-1(AUC=0.821)(P<0.05).Conclusion NLRP3 and Caspase-1 are influencing factors of intracranial atherosclerotic stenosis in patients with cerebral infarction.The combination of these two markers can effectively evaluate the degree of intracranial atherosclerotic stenosis in such patients.
