Changes in Hemorheological Indices and Their Correlation with Cerebral Thrombosis Formation in Elderly Patients with Cerebral Infarction

Abstract:Objective To analyze the changes in hemorheological indices and their correlation with cerebral thrombosis formation in elderly patients with cerebral infarction.Methods A total of 210 elderly patients with cerebral infarction who received diagnosis and treatment in the Department of Neurology,Xingtai Central Hospital from March 2023 to March 2025 were prospectively enrolled.After excluding 25 patients,185 cases were ultimately included.Based on the presence of new thrombus/embolus or definitive embolic signs(such as hyperdense vessel sign,thrombus sign,embolic truncation)in the responsible vessel,patients were divided into the new cerebral thrombus(NCT)group(with NCT,n=55)and the nNCT group(without NCT,n=130).Baseline data,plasma viscosity(PV),high-shear whole blood viscosity(HWBV),and low-shear whole blood viscosity(LWBV)were compared.Logistic regression was used to analyze risk factors for NCT formation in cerebral infarction patients.Receiver operating characteristic(ROC)curves were employed to evaluate the predictive value of PV,HWBV,and LWBV for NCT formation.Results No significant differences were observed between the NCT and nNCT groups in age,male,hyperlipidemia prevalence,or time from symptom onset(P>0.05).However,the NCT group showed significantly higher proportions of stroke history,diabetes,hypertension,and elevated PV,HWBV,and LWBV compared to the nNCT group(P<0.05).Logistic regression identified stroke history,diabetes,hypertension,and elevated PV,HWBV,and LWBV as relevant influencing factors for NCT formation(P<0.05).ROC curve analysis revealed AUC values of 0.835,0.685,and 0.668 for PV,HWBV,and LWBV,respectively,in predicting NCT;these were significantly lower than the AUC of 0.901 achieved by their combined assessment.Conclusion Elderly cerebral infarction patients exhibit significant alterations in hemorheological indices(PV,HWBV,LWBV).The degree of these changes correlates with radiologically evident thrombotic signs,providing ancillary insights for understanding pathophysiological characteristics of cerebral infarction in the elderly(e.g.,hypercoagulability,blood flow stasis).
