Serum SFRP2 and ANGPTL3 Levels in Patients with Acute Myocardial Infarction after Interventional Therapy and Their Relationship with Left Ventricular Remodeling and MACE

Abstract:Objective To explore the expression levels of serum secreted frizzled-related protein 2(SFRP2)and angiopoietin-like protein 3(ANGPTL3)in patients with acute myocardial infarction(AMI)after interventional therapy,and their relationship with left ventricular remodeling and major adverse cardiovascular events(MACE).Methods A total of 103 AMI patients who underwent interventional therapy at Shijiazhuang Emergency Center Internet Hospital from March 2023 to May 2024 were selected.Based on the occurrence of MACE within one year,they were divided into a non-MACE group(79 cases)and a MACE group(24 cases).General clinical data and left ventricular remodeling indices[including left atrial diameter(LAD),left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),and left ventricular mass index(LVMI)]were compared between the two groups.Pearson correlation analysis was used to examine the relationships between serum SFRP2,ANGPTL3,and left ventricular remodeling indices.ROC curve analysis was applied to evaluate the predictive value of serum SFRP2 and ANGPTL3 for MACE in AMI patients.Relative risk analysis was conducted to assess the risk of MACE in patients with high or low expression of SFRP2 and ANGPTL3.Results There were no statistically significant differences in age,gender,smoking,alcohol consumption,hypertension,diabetes,AMI subtype,systolic blood pressure,diastolic blood pressure,or body mass index between the two groups(P>0.05).Compared with the non-MACE group,the MACE group showed increased LAD[(43.58±4.62)mm vs(34.51±3.65)mm],LVEDD[(56.84±5.94)mm vs(45.81±4.92)mm],and LVMI(156.23±18.91 vs 108.95±11.65),and decreased LVEF[(45.26±4.92)%vs(62.15±6.48)%],with statistically significant differences(P<0.05).Pearson correlation analysis revealed that serum SFRP2 was negatively correlated with LAD,LVEDD,and LVMI(r=-0.526,-0.524,-0.538,P<0.001),and positively correlated with LVEF(r=0.531,P<0.001).Serum ANGPTL3 was positively correlated with LAD,LVEDD,and LVMI(r=0.498,0.501,0.532,P<0.001),and negatively correlated with LVEF(r=-0.528,P<0.05).The AUC values of serum SFRP2 and ANGPTL3 for predicting MACE in AMI patients were 0.867 and 0.853,respectively.The combined prediction of serum SFRP2 and ANGPTL3 yielded an AUC of 0.951,which was better than the individual predictions of SFRP2(Z=2.624,P=0.009)and ANGPTL3(Z=2.447,P=0.014).Conclusion In AMI patients,serum SFRP2 levels are decreased,while ANGPTL3 levels are increased.These levels are correlated with ventricular remodeling indicators and have certain auxiliary predictive value for assessing the occurrence of MACE.
