A Study on the Prevalence and Associated Factors of Suicide Attempts in First-Episode,Drug-Naïve,Single Patients with Major Depressive Disorder

Abstract:Objective To investigate the prevalence of suicide attempts and identify associated risk factors in first-episode,drug-naïve,single patients with major depressive disorder(MDD).Methods A total of 250 first-episode,drug-naïve,single MDD patients were selected from the First Hospital of Shanxi Medical University between January 2016 and December 2018.According to history of suicide attempts,they were divided into a suicide attempt group(n=60)and a non-suicide attempt group(n=190).The two groups were compared in terms of general information,scores on the 17-item Hamilton Depression Rating Scale(HAMD-17)and Hamilton Anxiety Rating Scale(HAMA),thyroid function,glucose and lipid metabolism indicators,and blood pressure.Binary multivariate logistic regression was used to analyze influencing factors for suicide attempts,and receiver operating characteristic(ROC)curves were used to evaluate the predictive performance of the risk factors.Results The prevalence of suicide attempts among the 250 first-episode,drug-naïve,single MDD patients was 24.00%(60/250).Compared with the non-suicide attempt group,the suicide attempt group showed higher HAMD-17 and HAMA scores,as well as higher levels of thyroid-stimulating hormone,thyroid peroxidase antibody(TPOAb),thyroglobulin antibody,fasting blood glucose,total cholesterol,triglyceride,low-density lipoprotein cholesterol,systolic blood pressure,and diastolic blood pressure(all P<0.05),while high-density lipoprotein cholesterol level was lower(P<0.05).Binary multivariate logistic regression analysis showed that HAMA score(OR=1.255,95%CI:1.089-1.447),TPOAb level(OR=1.004,95%CI:1.001-1.006),and systolic blood pressure(OR=1.104,95%CI:1.046-1.166)were independent risk factors for suicide attempts.ROC curve analysis showed that the combination of HAMA score,TPOAb,and systolic blood pressure predicted suicide attempts with an area under the curve of 0.837.Conclusion The incidence of suicide attempts is significantly elevated in first-episode,drug-naïve,single MDD patients.High HAMA score,elevated TPOAb level,and increased systolic blood pressure are influencing factors for suicide attempts.
