A Multidimensional Evaluation of SGLT2i Combined with Conventional Therapy on Glucolipid Metabolism and Nephroprotective Effects in DKD Patients with Hyperlipidemia

Abstract:Objective To investigate the influence of sodium-glucose cotransporter-2 inhibitor(SGLT2i)combined with conventional therapy on glucolipid metabolism and renal function in patients with diabetic kidney disease(DKD)complicated with hyperlipidemia.Methods A total of 120 patients with DKD complicated with hyperlipidemia admitted to Minhang Hospital Affiliated to Fudan University from January to December 2024 were selected and divided into a conventional group(60 cases)and a combination group(60 cases)according to the random number table method.The conventional group received conventional treatment,while the combination group received SGLT2i plus conventional treatment.Both groups were treated for 6 months.The lipid metabolism indicators[high-density lipoprotein cholesterol(HDL-C),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),and triglycerides(TG)],urine albumin-to-creatinine ratio(UACR),glucose metabolism indicators[glycated hemoglobin(HbA1c),fasting blood glucose(FPG),and 2-hour postprandial blood glucose(2 hPG)],and renal function indicators[uric acid(UA),serum creatinine(Scr),estimated glomerular filtration rate(eGFR),and blood urea nitrogen(BUN)]were compared before and after treatment between the two groups.Changes in body weight and the occurrence of adverse reactions during the treatment period were also compared between the two groups.Results After treatment,the levels of LDL-C[(2.79±0.73)mmol/L],TC[(3.24±0.38)mmol/L],TG[(2.08±0.27)mmol/L],HbA1c[(5.08±1.03)%],FPG[(6.22±1.37)mmol/L],2 hPG[(6.21±1.37)mmol/L],UACR[(167.95±15.84)mg/g],UA[(344.51±10.63)μmol/L],Scr[(70.65±5.94)μmol/L],and BUN[(6.23±1.03)mmol/L]in the combination group were lower than those in the conventional group[(3.47±0.76)mmol/L,(3.68±0.39)mmol/L,(2.39±0.26)mmol/L,(5.68±1.01)%,(6.74±1.23)mmol/L,(6.82±1.37)mmol/L,(189.97±16.74)mg/g,(363.58±12.84)μmol/L,(75.35±5.84)μmol/L,(7.58±1.02)mmol/L],while the levels of HDL-C[(1.49±0.24)mmol/L]and eGFR[(39.85±6.33)mL/(min·1.73 m²)]were higher than those in the conventional group[(1.21±0.27)mmol/L,(36.58±6.84)mL/(min·1.73 m²)],with statistically significant differences(P<0.05).After treatment,the body weight of the combination group was lower than that before treatment and also lower than that of the conventional group(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion SGLT2i combined with conventional therapy can improve glucolipid metabolism in DKD patients with hyperlipidemia,with a significant protective effect on renal function and good safety.
