Effect of Glucocorticoids on Lymphocytes in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease

Abstract:Objective To explore the effect of glucocorticoids on lymphocytes in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 60 AECOPD patients admitted to Rugao Branch Hospital of Nantong University from January 2023 to March 2024 were selected and divided into a control group(n=30,receiving conventional treatment)and an experimental group(n=30,receiving conventional treatment plus glucocorticoids)according to the treatment regimen.The following parameters were compared between the two groups before and after treatment:T lymphocyte subsets(CD3+,CD4+,CD4+/CD8+),B lymphocytes(CD19+cells),and NK cell levels;pulmonary function[forced expiratory volume in one second as a percentage of predicted value(FEV1%),ratio of forced expiratory volume in one second to forced vital capacity(FEV1/FVC)];arterial partial pressure of carbon dioxide,arterial partial pressure of oxygen;inflammatory factor levels(C-reactive protein,interleukin-6,tumor necrosis factor-α);and the incidence of adverse reactions.Results After treatment,the experimental group showed lower levels of CD4+,CD4+/CD8+,CD19+cells,and NK cells compared to the control group(P<0.05).The experimental group also had higher FEV1%and FEV1/FVC values after treatment than the control group(P<0.05).Compared with the control group,the experimental group exhibited lower arterial partial pressure of carbon dioxide,lower inflammatory factor levels,and higher arterial partial pressure of oxygen after treatment(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Glucocorticoids can reduce lymphocyte levels,improve pulmonary function,increase arterial partial pressure of oxygen,and decrease arterial partial pressure of carbon dioxide and inflammatory factor levels in AECOPD patients,with a good safety profile.
