Effect of Wenyang Bushen Paste on Regulating the Inflammatory Pathway in Elderly Patients with Benign Prostatic Hyperplasia of Kidney-Yang Deficiency Type and Frequent Nocturia:A Randomized Controlled Trial

Abstract:Objective To explore the regulatory effect of Wenyang Bushen Paste on key factors of the inflammatory pathway and its improvement effect on clinical symptoms in elderly patients with benign prostatic hyperplasia(BPH)of kidney-yang deficiency type with frequent nocturia through a prospective randomized controlled trial.Methods A total of 120 elderly patients with BPH and frequent nocturia of kidney-yang deficiency syndrome admitted to Tangshan Hospital of Traditional Chinese Medicine from January 2023 to September 2024 were selected and divided into a study group and a control group using a random number table,with 60 cases in each group.The control group received only basic treatment,while the study group received additional acupoint application of Wenyang Bushen Paste.The treatment course for both groups was 4 weeks.The therapeutic efficacy,TCM syndrome score,symptom-related indicators[nocturnal urination frequency,maximum urinary flow rate(Qmax),post-void residual urine volume(PVR),Pittsburgh Sleep Quality Index(PSQI)],serum inflammatory pathway markers[matrix metalloproteinase-9(MMP-9),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)],quality of life[Chinese version of the Nocturia Quality of Life Scale(N-QOL)],and adverse reactions were compared between the two groups.Results After 4 weeks of treatment,the total effective rate in the study group was higher than that in the control group(P<0.05).The TCM syndrome scores in both groups decreased compared to those before treatment(P<0.05),and the score in the study group was lower than that in the control group(P<0.05).After 4 weeks of treatment,the nocturnal urination frequency,PVR,and PSQI scores in the study group were lower than those in the control group(P<0.05),while Qmax was higher than that in the control group(P<0.05).After 4 weeks of treatment,the levels of MMP-9,TNF-α,and IL-6 in the study group were lower than those in the control group(P<0.05).The N-QOL score in the study group was higher than that in the control group(P<0.05).No adverse reactions occurred in either group during the study.Conclusion Acupoint application of Wenyang Bushen Paste effectively improves urodynamic parameters,sleep quality,and quality of life in elderly patients with BPH of kidney-yang deficiency type and frequent nocturia by downregulating the MMP-9/TNF-α/IL-6 inflammatory pathway,providing a new non-pharmacological intervention strategy based on inflammatory regulation mechanisms for clinical practice.
