Occurrence of Peripherally Inserted Central Catheter-Related Venous Thrombosis in Patients with Malignant Lymphoma Undergoing Chemotherapy and Construction of a Predictive Model for Its Influencing Factors

Abstract:Objective To investigate the occurrence of peripherally inserted central catheter(PICC)-related venous thrombosis in patients undergoing chemotherapy for malignant lymphoma and to construct a prediction model for influencing factors.Methods A total of 109 patients with malignant lymphoma receiving chemotherapy at Tangdu Hospital of Air Force Military Medical University from April 2022 to April 2024 were retrospectively selected as the study subjects.Based on the occurrence of PICC-related venous thrombosis,the patients were divided into a thrombosis group(21 cases)and a non-thrombosis group(88 cases).General data and catheter-related information were compared between the two groups.Statistically significant single factors were included in multivariate logistic regression analysis to identify influencing factors for PICC-related venous thrombosis,and a prediction model was constructed.The predictive value of the model for PICC-related venous thrombosis was analyzed using the receiver operating characteristic(ROC)curve.Results All 109 patients with malignant lymphoma receiving chemotherapy underwent PICC,and the incidence of PICC-related venous thrombosis was 19.27%(21/109).The comparison between the two groups in terms of catheter vein location,catheter tip position,catheter diameter/vein diameter ratio,number of previous central venous punctures,and venous blood flow velocity showed statistically significant differences(P<0.05).Multivariate logistic regression analysis indicated that catheter tip position(OR=1.192,95%CI:1.056-1.347),catheter diameter/vein diameter ratio(OR=1.012,95%CI:1.008-1.016),number of previous central venous punctures(OR=1.394,95%CI:1.220-1.592),and venous blood flow velocity(OR=0.931,95%CI:0.888-0.975)were influencing factors for PICC-related venous thrombosis(P<0.05).ROC curve analysis showed that the area under the curve of the prediction model was 0.959,with a sensitivity of 100.00%,specificity of 86.36%,and 95%CI:0.903-0.988(P<0.05).Conclusion The occurrence of PICC-related venous thrombosis in patients with malignant lymphoma receiving chemotherapy is associated with catheter tip position,venous blood flow velocity,number of previous central venous punctures,and catheter diameter/vein diameter ratio.The prediction model constructed based on these factors demonstrates high predictive performance.
