Value of High-Resolution Chest CT Combined with Peripheral Blood actinin-4 and CDH5 in the Characterization of Pulmonary Nodules

Abstract:Objective To explore the value of high-resolution computed tomography(HRCT)combined with peripheral blood actinin-4 and cadherin 5(CDH5)in evaluating the benign and malignant nature of solitary pulmonary nodules.Methods A total of 188 patients with solitary pulmonary nodules admitted to Guangyuan Central Hospital from January 2024 to February 2025 were retrospectively selected.All patients underwent HRCT examination and pathological biopsy.According to the pathological biopsy results,they were divided into a benign nodule group(62 cases)and a malignant nodule group(126 cases).The HRCT imaging features and peripheral blood actinin-4 and CDH5 levels were compared between the two groups.Binary Logistic stepwise regression analysis was used to analyze the influencing factors of benign and malignant solitary pulmonary nodules,and ROC curve analysis was used to evaluate the diagnostic value of HRCT imaging features combined with peripheral blood actinin-4 and CDH5 in distinguishing benign and malignant solitary pulmonary nodules.Results Among the 188 patients with solitary pulmonary nodules,62 had benign nodules,with pathological diagnosis mainly tuberculosis in 36 cases(58.06%),and 126 had malignant nodules,with pathological diagnosis mainly adenocarcinoma in 91 cases(72.22%).The rates of smoking,HRCT imaging features(ill-defined borders,lobulation,spiculation,bronchial truncation,vascular convergence,and pleural indentation),and levels of actinin-4 and CDH5 in the benign nodule group were lower than those in the malignant nodule group(P<0.05).Binary Logistic stepwise regression analysis showed that smoking,ill-defined borders,lobulation,spiculation,bronchial truncation,vascular convergence,pleural indentation,actinin-4,and CDH5 were influencing factors for benign and malignant solitary pulmonary nodules(OR=3.732,95%CI:1.387-10.042,P=0.009;OR=10.237,95%CI:3.227-32.474,P<0.001;OR=5.720,95%CI:1.989-16.452,P=0.001;OR=8.356,95%CI:2.356-29.641,P=0.001;OR=6.980,95%CI:2.384-20.432,P<0.001;OR=3.529,95%CI:1.296-9.608,P=0.014;OR=4.655,95%CI:1.499-14.453,P=0.008;OR=1.336,95%CI:1.030-1.734,P=0.030;OR=3.264,95%CI:1.405-7.582,P=0.006).ROC curve analysis showed that the AUC for the combined diagnosis of malignant solitary pulmonary nodules was 0.961,while the AUC for HRCT imaging features,actinin-4,and CDH5 alone were 0.801,0.845,and 0.885,respectively.The combined diagnosis was significantly better than any single diagnostic method(P<0.05).Conclusion HRCT imaging features,actinin-4,and CDH5 levels show significant differences between benign and malignant solitary pulmonary nodules.The combined diagnosis has higher reference value in the clinical qualitative diagnosis of solitary pulmonary nodules.
