Analysis of Risk Factors and Construction of A Prediction Model for Postoperative Malnutrition in Patients with Laryngeal Cancer

Abstract:Objective To analyze the risk factors for postoperative malnutrition in patients with laryngeal cancer and to construct a risk prediction model.Methods A total of 202 patients with laryngeal cancer treated at the First Affiliated Hospital of Nanjing Medical University from January 2022 to July 2024 were retrospectively selected and divided into a malnutrition group(n=64)and a well-nourished group(n=138)based on the occurrence of postoperative malnutrition.Multivariate logistic regression was used to analyze the influencing factors of malnutrition;a nomogram model for malnutrition was constructed,and its predictive value was evaluated using the receiver operating characteristic curve.Results The malnutrition group had a higher proportion of patients aged≥60 years,with a monthly household income per capita<3 000 yuan,clinical stage(Ⅲ-Ⅳ),total laryngectomy,neck lymph node dissection,and postoperative complications,while body mass index(BMI),hemoglobin,total protein,and albumin levels were lower than those in the well-nourished group(P<0.05).Multivariate analysis showed that age≥60 years(OR=1.998,95%CI:1.415-2.821),low BMI(OR=0.490,95%CI:0.345-0.694),clinical stage(Ⅲ-Ⅳ)(OR=2.361,95%CI:1.583-3.521),and low albumin level(OR=0.331,95%CI:0.213-0.516)were independent risk factors for postoperative malnutrition in patients with laryngeal cancer(P<0.05).The constructed nomogram model for predicting postoperative malnutrition in laryngeal cancer patients showed a consistency index(C-index)of 0.873(95%CI:0.821-0.925).The area under the curve(95%CI)for predicting postoperative malnutrition in laryngeal cancer patients was 0.881(0.829-0.933),with a specificity of 0.682 and a sensitivity of 0.916.Conclusion The nomogram model for predicting postoperative malnutrition in patients with laryngeal cancer,based on age,BMI,clinical stage,and albumin level,demonstrates good predictive performance and clinical application value.
