A Nomogram for Predicting the Risk of New-Onset Lower Extremity Deep Vein Thrombosis After Internal Fixation of Intertrochanteric Femoral Fractures

Abstract:Objective To analyze the risk factors for new-onset lower extremity deep vein thrombosis(LEDVT)after internal fixation in patients with intertrochanteric femoral fracture(IFF)and to establish a nomogram prediction model.Methods A total of 228 IFF patients who underwent internal fixation at the First Affiliated Hospital of Hebei North University from May 2022 to December 2024 were retrospectively selected.According to the occurrence of new-onset LEDVT within 3 months after surgery,they were divided into the LEDVT group(52 cases)and the non-LEDVT group(176 cases).Multivariate Logistic stepwise regression was used to analyze the influencing factors for new-onset LEDVT after internal fixation in IFF patients.A nomogram model was developed to predict the risk of new-onset LEDVT after internal fixation,and its predictive performance was evaluated using ROC curve analysis.Results The LEDVT group showed higher age,prevalence of diabetes,time from injury to admission,use of general anesthesia,operation time,intraoperative blood loss,time to first ambulation after surgery,and D-dimer levels compared to the non-LEDVT group(P<0.05).Multivariate Logistic stepwise regression analysis indicated that advanced age,diabetes,general anesthesia,longer time to first postoperative ambulation,and high D-dimer level were risk factors for new-onset LEDVT after internal fixation in IFF patients(OR=1.996,95%CI:1.425-2.796,P<0.01;OR=2.164,95%CI:1.512-3.098,P<0.01;OR=2.519,95%CI:1.689-3.758,P<0.01;OR=2.330,95%CI:1.603-3.388,P<0.01;OR=3.019,95%CI:1.939-4.702,P<0.01).The AUC of the nomogram for predicting new-onset LEDVT after internal fixation was 0.904,with a sensitivity of 92.45%and a specificity of 68.71%.Conclusion The nomogram model based on risk factors for new-onset LEDVT after internal fixation in IFF patients demonstrates good predictive performance and can serve as an important tool for individualized clinical risk assessment.
