Association Between Serum SF,PAPP-A,and 25(OH)D Levels in Women with Gestational Diabetes Mellitus and the Occurrence of Neonatal Hypoglycemia

Abstract:Objective To analyze the relationship between serum ferritin(SF),pregnancy-associated plasma protein A(PAPP-A),and 25-hydroxyvitamin D[25(OH)D]levels in pregnant women with gestational diabetes mellitus(GDM)and the occurrence of neonatal hypoglycemia.Methods A total of 122 pregnant women with GDM who were admitted to Xingtai Central Hospital from August 2023 to December 2024 were selected as study subjects.They were divided into an adverse outcome group(31 cases)and a good outcome group(91 cases)based on whether neonatal hypoglycemia occurred.The levels of SF,PAPP-A,and 25(OH)D were compared between the two groups.A logistic regression model was used to analyze whether SF,PAPP-A,and 25(OH)D levels in GDM pregnant women were independent influencing factors for neonatal hypoglycemia.The receiver operating characteristic(ROC)curve was employed to evaluate the predictive value of SF,PAPP-A,and 25(OH)D levels individually and in combination for neonatal hypoglycemia.Results Compared with the good outcome group,the adverse outcome group had a higher SF level(P<0.05)and lower PAPP-A and 25(OH)D levels(P<0.05).Logistic regression analysis showed that SF,PAPP-A,and 25(OH)D levels in GDM pregnant women were independent influencing factors for neonatal hypoglycemia(P<0.05).ROC curve analysis revealed that the area under the curve(AUC)of SF,PAPP-A,and 25(OH)D for predicting neonatal hypoglycemia were 0.871,0.817,and 0.823,respectively;the sensitivities were 77.42%,70.97%,and 67.74%;and the specificities were 80.22%,90.11%,and 83.52%.The combination of SF,PAPP-A,and 25(OH)D yielded an AUC of 0.978 for predicting neonatal hypoglycemia,with a sensitivity of 90.32%and a specificity of 97.80%.Conclusion The levels of SF,PAPP-A,and 25(OH)D in pregnant women with GDM are closely related to the occurrence of neonatal hypoglycemia.The combination of these three indicators has high predictive value for neonatal hypoglycemia.
