Therapeutic Outcome and Risk Factors for Spinal Stability of Posterior Pedicle Screw Fixation through the Fractured Vertebra in Thoracolumbar Fractures

Abstract:Objective To explore the therapeutic effect of posterior pedicle screw fixation through the fractured vertebra on patients with thoracolumbar spine fractures and to analyze the factors affecting spinal stability.Methods A retrospective study was conducted on 103 patients with thoracolumbar spine fractures treated with posterior pedicle screw fixation through the fractured vertebra(observation group)and 97 patients treated with traditional pedicle screw fixation across the fractured vertebra(control group)at Shanghai Second People's Hospital from April 2020 to December 2023.The clinical efficacy was compared between the two groups.Patients in the observation group were followed up for 1 year and divided into a good spinal stability group(87 cases)and a poor spinal stability group(16 cases)based on their spinal stability.General data were compared between the two groups,and Logistic regression analysis was used to identify factors affecting spinal stability in patients with thoracolumbar spine fractures after posterior pedicle screw fixation through the fractured vertebra.Results The total effective rate in the observation group was 95.15%,significantly higher than that in the control group(84.54%)(P<0.05).Compared with the good spinal stability group,the poor spinal stability group had a higher proportion of burst fractures,parallel screw placement,postoperative infection,and concurrent osteoporosis,as well as older age(P<0.05).Logistic regression analysis showed that the factors affecting spinal stability in patients with thoracolumbar spine fractures after posterior pedicle screw fixation through the fractured vertebra were burst fracture(OR=1.441,95%CI:1.136-1.826),concurrent osteoporosis(OR=1.369,95%CI:1.034-1.812),age(OR=1.065,95%CI:1.022-1.110),postoperative infection(OR=1.293,95%CI:1.020-1.639),and parallel screw placement(OR=1.301,95%CI:1.065-1.589)(P<0.05).Conclusion Posterior pedicle screw fixation through the fractured vertebra has a good therapeutic effect on patients with thoracolumbar spine fractures.Burst fracture,concurrent osteoporosis,age,postoperative infection,and parallel screw placement are factors affecting postoperative spinal stability in these patients.
