Abstract:Objective To analyze the predictive value of serum chemokine C-C motif ligand 3(CCL3)and signal peptide-CUB-epidermal growth factor domain-containing protein 1(SCUBE1)for deep vein thrombosis(DVT)after hip and knee arthroplasty. Methods A total of 110 patients who underwent hip or knee arthroplasty at Jianyang City People's Hospital from January 2022 to January 2024 were retrospectively selected as the study subjects.Based on lower limb venography results at 1 month after surgery, patients were divided into a DVT group(48 cases)and a non-DVT group(62 cases).Clinical data,as well as preoperative and postoperative day 1 levels of CCL3,SCUBE1,and coagulation parameters[activated partial thromboplastin time, prothrombin time, and fibrinogen(FIB)],were compared between the two groups.Spearman correlation analysis was used to analyze the correlations between CCL3,SCUBE1,and coagulation parameters.Multivariate logistic regression analysis was employed to identify risk factors for lower limb DVT after hip and knee arthroplasty.Receiver operating characteristic(ROC)curves were plotted to evaluate the predictive performance of CCL3 and SCUBE1 levels for lower limb DVT.Results The proportion of patients with postoperative bed rest time≥3 days was higher in the DVT group than in the non-DVT group(P<0.05).On postoperative day 1,levels of CCL3,SCUBE1,and FIB were elevated in the DVT group(P<0.05),and these levels were higher in the DVT group than in the non-DVT group(P<0.05).Spearman correlation analysis showed that CCL3 and SCUBE1 levels were significantly positively correlated with FIB(r=0.572,0.591,P<0.05).Multivariate logistic regression analysis indicated that high levels of CCL3,SCUBE1,and FIB were risk factors for lower limb DVT after hip and knee arthroplasty(P<0.05).ROC curve analysis revealed that the area under the curve for the combined prediction of CCL3 and SCUBE1 for lower limb DVT was 0.950,which was higher than that of CCL3 alone(0.891)or SCUBE1(0.879).Conclusion Serum levels of CCL3 and SCUBE1 are elevated in patients with lower limb DVT after hip and knee arthroplasty, and the combination of CCL3 and SCUBE1 has higher predictive value for lower limb DVT.
