The Correlation Between Serum CA125,SOD,and Apelin Expression and Sex Hormone Levels in Patients with Premature Ovarian Insufficiency and Their Predictive Value for Clinical Outcomes of IVF-ET

Abstract:Objective To investigate the correlation between the expression of serum carbohydrate antigen 125(CA125),superoxide dismutase(SOD),and adipokine(Apelin)with sex hormone levels in patients with premature ovarian insufficiency and their predictive value for the clinical outcome of in vitro fertilization and embryo transfer(IVF-ET).Methods From July 2022 to July 2024,patients with premature ovarian insufficiency who underwent IVF-ET at the First Affiliated Hospital of Xinjiang Medical University were selected as the ovarian insufficiency group(132 cases),while individuals with healthy ovarian function and premenopausal status examined during the same period were included as the healthy ovarian function group(132 cases).The ovarian insufficiency group was further divided into a successful pregnancy group(38 cases)and a failed pregnancy group(94 cases)based on IVF-ET pregnancy outcomes.Serum levels of CA125,SOD,and Apelin were measured using enzyme-linked immunosorbent assay.Pearson correlation analysis was used to analyze the relationship between serum CA125,SOD,and Apelin levels and sex hormones in patients with premature ovarian insufficiency.The relative risk of pregnancy failure in patients with different levels of CA125,SOD,and Apelin was compared.The predictive value of CA125,SOD,and Apelin for pregnancy failure in patients with premature ovarian insufficiency was analyzed using ROC curves,and the predictive model was internally validated.Results The ovarian insufficiency group had longer menstrual cycle lengths and shorter menstrual durations than the healthy ovarian function group[(2.12±0.53)months vs(1.01±0.14)months,P<0.01;(3.24±0.97)d vs(6.38±0.85)d,P<0.01].The ovarian insufficiency group had higher serum CA125 and Apelin levels and lower SOD levels than the healthy ovarian function group[(3.29±0.81)U/mL vs(0.85±0.17)U/mL,P<0.01;(535.13±152.45)pg/mL vs(266.49±64.31)pg/mL,P<0.01;(10.92±2.43)ng/mL vs(15.37±2.08)ng/mL,P<0.01].In patients with premature ovarian insufficiency,serum CA125 and Apelin were positively correlated with luteinizing hormone and follicle-stimulating hormone(P<0.01)and negatively correlated with estradiol and anti-Müllerian hormone(P<0.01).SOD was negatively correlated with luteinizing hormone and follicle-stimulating hormone(P<0.01)and positively correlated with estradiol and anti-Müllerian hormone(P<0.01).The pregnancy failure group had higher serum CA125 and Apelin levels and lower SOD levels than the successful pregnancy group(P<0.01).The relative risk of pregnancy failure in patients with high expression of CA125,SOD,and Apelin was 1.343,0.733,and 1.266 times that of patients with low expression of CA125,SOD,and Apelin,respectively(P<0.05).The area under the curve for the combined prediction of pregnancy failure in patients with premature ovarian insufficiency was 0.936,which was significantly better than the individual predictions of CA125,SOD,and Apelin(0.859,0.863,and 0.831,respectively;P<0.05).The Hosmer-Lemeshow test showed that the prediction results of the pregnancy failure prediction model constructed by Logistic regression combining CA125,SOD,and Apelin were highly consistent with the actual situation.Conclusion Serum CA125 and Apelin levels are elevated,and SOD levels are reduced in patients with premature ovarian insufficiency.The combination of these markers has high value in predicting pregnancy failure in patients with premature ovarian insufficiency.
