Value of Serum MCP-1 and MIP-1α for the Evaluation of Cognitive Function in Elderly Patients with First-Episode Depression

Abstract:Objective To explore the value of serum monocyte chemoattractant protein-1(MCP-1)and macrophage inflammatory protein-1α(MIP-1α)in assessing cognitive function in elderly patients with first-episode depression.Methods A total of 142 elderly patients with first-episode depression admitted to Jilin Provincial Neuropsychiatric Hospital from December 2021 to December 2024 were selected.Based on cognitive function assessment,they were divided into a cognitive impairment group(43 cases)and a cognitively normal group(99 cases).The value of serum MCP-1 and MIP-1α in assessing cognitive function in elderly patients with first-episode depression was analyzed using ROC curves.Multivariate Logistic stepwise regression was used to analyze the influencing factors of cognitive function in elderly patients with first-episode depression.Results The levels of serum MCP-1 and MIP-1α in the cognitive impairment group were higher than those in the cognitively normal group(P<0.01).The AUC of the combination of MCP-1 and MIP-1α for assessing cognitive function in elderly patients with first-episode depression was 0.908,which was significantly superior to that of MCP-1 and MIP-1α alone(0.757 and 0.856,respectively;P<0.01).The cognitive impairment group had higher age,Hamilton Depression Rating Scale-17 score,homocysteine,C-reactive protein,tumor necrosis factor-α,interleukin-6,as well as higher proportions of diabetes and living alone compared to the cognitively normal group(P<0.05).Multivariate Logistic stepwise regression analysis showed that living alone,elevated Hamilton Depression Rating Scale-17 score,elevated homocysteine,MCP-1≥41.98 ng/L,and MIP-1α≥22.97 ng/L were independent risk factors for cognitive function in elderly patients with first-episode depression(OR=2.852,95%CI:1.403-5.798,P=0.004;OR=2.022,95%CI:1.159-3.528,P=0.013;OR=2.121,95%CI:1.312-3.429,P=0.002;OR=2.237,95%CI:1.325-3.775,P=0.003;OR=2.394,95%CI:1.502-3.817,P<0.01).Conclusion Elevated serum MCP-1 and MIP-1α levels are closely associated with cognitive function in elderly patients with first-episode depression.Detecting changes in serum MCP-1 and MIP-1α levels can significantly improve the assessment efficacy for cognitive impairment risk in these patients,providing a basis for early implementation of targeted intervention strategies,thereby reducing the risk of cognitive impairment and improving patients'quality of life.
