Effect of Autologous Plateletpheresis and Reinfusion on Thromboelastography Parameters and Early Prognosis in Patients Undergoing Acute Type A Aortic Dissection Surgery

Abstract:Objective To investigate the influence of intraoperative autologous plateletpheresis and reinfusion on thromboelastography parameters and early prognosis in patients undergoing acute type A aortic dissection(ATAAD)surgery.Methods A total of 94 patients who underwent ATAAD surgery at the Affiliated Hospital of Jining Medical University from January 2022 to December 2024 were retrospectively enrolled.According to whether autologous plateletpheresis and reinfusion was applied,they were divided into a control group(46 cases,receiving allogeneic platelet transfusion)and an observation group(48 cases,receiving autologous plateletpheresis and reinfusion).Thromboelastography parameters[clot reaction time(R value),clot formation time(K value),maximum amplitude(MA value)]and coagulation function indexes[fibrinogen(FIB),thrombin time(TT),activated partial thromboplastin time(APTT),prothrombin time(PT)]before surgery and 48 hours after surgery,as well as early prognosis and the incidence of postoperative complications,were compared between the two groups.Results Compared with the control group,the observation group at 48 hours after surgery showed significantly smaller R and K values,larger MA value,shorter PT,APTT,and TT,and higher FIB level,with statistically significant differences(P<0.05).Compared with the control group,the observation group had significantly shorter duration of mechanical ventilation,length of ICU stay,and hospital stay(P<0.05).The total incidence of postoperative complications in the observation group was 4.16%,significantly lower than that in the control group(17.40%),with a statistically significant difference(P<0.05).Conclusion Autologous plateletpheresis and reinfusion can better improve thromboelastography parameters and coagulation function,optimize early prognosis,and reduce the incidence of postoperative complications in patients undergoing ATAAD surgery.
