A Meta-Analysis of Survival Following Allogeneic versus Autologous Hematopoietic Stem Cell Transplantation for Relapsed/Refractory Peripheral T-Cell Lymphoma

Abstract:Objective This study systematically evaluated the overall survival(OS)of patients with relapsed or refractory peripheral T-cell lymphoma(R/R-PTCL)treated with allogeneic hematopoietic stem cell transplantation(allo-HSCT)or autologous hematopoietic stem cell transplantation(auto-HSCT),in order to compare the OS between allo-HSCT and auto-HSCT for R/R-PTCL.Methods Computerized searches were conducted in PubMed,Cochrane Library,Embase,Web of Science,OVID,Wanfang Database,and China National Knowledge Infrastructure.Literature meeting the inclusion criteria was collected,with the search timeframe spanning from the establishment of each database to June 1,2025.Two reviewers performed meta-analysis using StataMP 18 software in accordance with the Cochrane systematic review methodology.Results A total of 16 studies were included,among which one was a randomized controlled trial and 15 were retrospective studies,involving 1 699 patients.The 1-year,3-year,5-year,7-year,and 10-year OS rates for the auto-HSCT and allo-HSCT groups were 70%(95%CI:0.67-0.73,P=0.001)and 64%(95%CI:0.61-0.67,P<0.001);54%(95%CI:0.50-0.57,P<0.001)and 52%(95%CI:0.49-0.56,P<0.001);45%(95%CI:0.41-0.49,P=0.076)and 49%(95%CI:0.45-0.53,P=0.951);40%(95%CI:0.35-0.45,P=0.160)and 38%(95%CI:0.30-0.46,P=0.140);40%(95%CI:0.34-0.46,P=0.150)and 38%(95%CI:0.29-0.47,P=0.247),respectively.Conclusion Patients with R/R-PTCL who underwent allo-HSCT and auto-HSCT had similar survival outcomes.
