Serum Nucleic Acid Levels in Patients with Chronic Hepatitis B and Their Relationship with Liver Inflammation

Abstract:Objective To detect the levels of serum hepatitis B virus DNA(HBV DNA)and hepatitis B virus RNA(HBV RNA)in patients with chronic hepatitis B and analyze their relationship with liver inflammation.Methods A total of 128 patients with chronic hepatitis B admitted to Cangzhou Third Hospital from January 2022 to January 2023 were selected as the study subjects.All patients underwent liver puncture,and based on the results,liver inflammatory activity was graded into group G0(29 cases),group G1(25 cases),group G2(27 cases),group G3(26 cases),and group G4(21 cases).The levels of serum nucleic acids HBV RNA,HBV DNA,alanine aminotransferase(ALT),aspartate aminotransferase(AST),gamma-glutamyl transferase(GGT),and alpha-fetoprotein(AFP)were compared among patients with different grades of liver inflammatory activity(G0-G4)and stages of liver fibrosis(S0-S4).Linear regression and Spearman correlation analysis were used to examine the relationships of HBV RNA and HBV DNA with liver inflammation,the degree of liver fibrosis,and the levels of ALT,AST,GGT,and AFP.A multivariate logistic regression model was used to construct a non-invasive diagnostic model for liver inflammation,and receiver operating characteristic(ROC)curve analysis was performed.Results The levels of HBV DNA,HBV RNA,ALT,AST,GGT,and AFP increased sequentially across the liver inflammatory activity grades(G0,G1,G2,G3,and G4 groups)and liver fibrosis stages(S0,S1,S2,S3,and S4 groups),with statistically significant differences in all pairwise comparisons(P<0.05).Correlation analysis showed that the expression levels of HBV DNA and HBV RNA were positively correlated with the degree of liver inflammation,the extent of liver fibrosis,and the levels of ALT,AST,GGT,and AFP(P<0.05).Based on these indicators,a non-invasive diagnostic model for liver inflammation(Log P model)was constructed using multivariate logistic regression.ROC curve analysis indicated that the model had good diagnostic performance,with an area under the curve of 0.795(95%CI:0.684-0.853),a sensitivity of 0.753,and a specificity of 0.775.The Hosmer-Lemeshow test results showed that the model had a good fit(χ2=5.211,P=0.698).Conclusion Serum HBV RNA and HBV DNA levels are positively correlated with liver inflammation in patients with chronic hepatitis B.The model constructed based on serum HBV RNA,HBV DNA,ALT,AST,GGT,and AFP exhibits good diagnostic efficacy for liver inflammation.
