The Application Value of Serum GTF2E2 and SPAG5 Levels in the Differential Diagnosis of Perimenopausal Endometrial Cancer and Endometrial Polyps

Abstract:Objective To explore the diagnostic value of serum general transcription factor ⅡE subunit beta(GTF2E2)and sperm-associated antigen 5(SPAG5)in distinguishing between endometrial cancer(EC)and endometrial polyps(EP)in perimenopausal women.Methods Patients with perimenopausal abnormal uterine bleeding who were treated at Yangzhou Maternal and Child Health Hospital from March 2023 to March 2025 were selected as the study group(82 cases).Based on histopathological diagnosis,they were divided into an EC group(38 cases)and an EP group(44 cases).Additionally,women who underwent health examinations at the Yangzhou Maternal and Child Health Hospital during the same period were selected as the control group(50 cases).Serum levels of GTF2E2 and SPAG5 were measured.ROC curves were used to analyze the differential diagnostic value of serum GTF2E2 and SPAG5 expression for EC versus EP.Multivariate Logistic regression was used to analyze factors influencing the occurrence of EC in perimenopausal women.Results The expression levels of serum GTF2E2 and SPAG5 in the study group were significantly higher than those in the control group(P<0.01).The EC group had significantly higher serum GTF2E2 and SPAG5 levels than the EP group(P<0.01).Among EC patients,those with lymph node metastasis,poor differentiation,and FIGO stage Ⅲ-Ⅳ had significantly higher rates of high expression of GTF2E2 and SPAG5 compared to those without lymph node metastasis,moderate to high differentiation,and FIGO stage Ⅰ-Ⅱ(P<0.05).Multivariate Logistic regression analysis showed that elevated expression of GTF2E2 and SPAG5 were independent risk factors for EC in patients with perimenopausal abnormal uterine bleeding(OR=4.330,95%CI:1.916-9.785,P<0.01;OR=3.951,95%CI:1.997-7.815,P<0.01).The AUC of serum GTF2E2 and SPAG5 alone for diagnosing EC in perimenopausal abnormal uterine bleeding patients were 0.841 and 0.735,respectively.The combined diagnostic AUC of the two markers was 0.924,which was significantly better than that of either marker alone(P<0.05).Conclusion Elevated serum levels of GTF2E2 and SPAG5 are independent risk factors for EC in patients with perimenopausal abnormal uterine bleeding.The combination of both markers has significant value in the differential diagnosis between EC and EP in perimenopausal women.
