Analysis of Risk Factors and Construction of A Nomogram Model for Postoperative Elevated Intraocular Pressure in Elderly Cataract Patients

Abstract:Objective To analyze the risk factors for postoperative intraocular pressure elevation in elderly cataract patients and construct a nomogram model.Methods A total of 206 elderly cataract patients admitted to the First Affiliated Hospital of Air Force Medical University from March 2023 to February 2025 were selected as the study subjects.They were divided into the elevated intraocular pressure group(n=25)and the normal intraocular pressure group(n=181)based on whether postoperative intraocular pressure elevation occurred.Multivariate logistic stepwise regression analysis was used to identify the influencing factors of postoperative intraocular pressure elevation in elderly cataract patients;a nomogram model was constructed,and its predictive value was evaluated using the receiver operating characteristic(ROC)curve.Results The elevated intraocular pressure group had higher age,and higher proportions of diabetes,lens nuclear hardness grade(Ⅲ-Ⅵ),viscoelastic agent retention,intraoperative complications,and high myopia than the normal intraocular pressure group(P<0.05).The disease duration and operation time were longer in the elevated intraocular pressure group than in the normal intraocular pressure group(P<0.05).Multivariate logistic stepwise regression analysis showed that diabetes,intraoperative complications,high myopia,and longer operation time were risk factors for postoperative intraocular pressure elevation in elderly cataract patients(P<0.05).The consistency index of the nomogram model was 0.769(95%CI:0.718-0.820);the calibration curve showed that its slope was close to 1.ROC curve analysis showed that the area under the curve(95%CI)of the nomogram model for predicting intraocular pressure elevation was 0.906(0.855-0.957),with a specificity of 68.74%and a sensitivity of 91.55%.Conclusion The influencing factors for postoperative intraocular pressure elevation in elderly cataract patients include diabetes,intraoperative complications,high myopia,and longer operation time.The nomogram model constructed based on these factors has high predictive accuracy.
