Comparative Study of Neoadjuvant Chemotherapy with Versus Without Recombinant Human Endostatin for Olfactory Neuroblastoma

Abstract:Objective To compare the efficacy of neoadjuvant chemotherapy(NACT)with versus without recombinant human endostatin in the treatment of olfactory neuroblastoma(ONB).Methods A retrospective analysis was conducted on the clinical data of 44 patients with pathologically confirmed ONB who received NACT at Beijing Tongren Hospital,Capital Medical University,from November 2012 to June 2022.Based on whether recombinant human endostatin was combined,they were divided into a combined group(24 cases)and a non-combined group(20 cases).The therapeutic efficacy after NACT,tumor volume,surgical approach,as well as alanine aminotransferase(ALT)levels,white blood cell count,platelet count,and hemoglobin levels before and after NACT were compared between the two groups.Results The overall response rate to NACT in the 44 patients was 50.00%(22/44).The percentage of cases with reduced tumor volume was 68.18%(30/44).A total of 31 cases(70.45%)underwent endoscopic resection,while 13 cases(29.55%)underwent craniotomy.Intergroup comparison showed that the response rate was 41.67%(10/24)in the combined group and 60.00%(12/20)in the non-combined group,with no statistically significant difference(P>0.05).The percentage of cases with reduced tumor volume was 66.67%(16/24)in the combined group and 70.00%(14/20)in the non-combined group,with no statistically significant difference(P>0.05).Compared with pre-NACT levels within each group,ALT increased after NACT in both groups(P<0.05),while white blood cell count,platelet count,and hemoglobin levels decreased(P<0.05).However,there were no statistically significant differences between the two groups in post-NACT ALT,white blood cell count,platelet count,or hemoglobin levels(all P>0.05).The endoscopic surgery rate was 75.00%(18/24)and the craniotomy rate was 25.00%(6/24)in the combined group,compared with 65.00%(13/20)and 35.00%(7/20)in the non-combined group,with no statistically significant difference(P>0.05).Conclusions NACT can effectively treat ONB,reduce tumor volume,and increase the rate of endoscopic surgery.The addition of recombinant human endostatin did not significantly improve efficacy or optimize the surgical approach,but its safety profile was acceptable.
