Analysis of the Efficacy and Prognostic Factors of Transcatheter Arterial Chemoembolization Combined with Immunotherapy in the Treatment of Primary Liver Cancer

Abstract:Objective To analyze the efficacy and prognostic factors of transcatheter arterial chemoembolization(TACE)combined with immunotherapy in the treatment of primary liver cancer(PLC).Methods A total of 103 patients with PLC admitted to Beijing Aerospace General Hospital from January 2022 to June 2024 were retrospectively selected.Based on the treatment method,47 patients treated with TACE were assigned to the TACE group,and 56 patients treated with TACE combined with camrelizumab were assigned to the combination group.The clinical efficacy[objective response rate(ORR)],liver function[alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBil)],and tumor markers[alpha-fetoprotein(AFP),hepatocyte growth factor(HGF),carcinoembryonic antigen(CEA)]were compared between the two groups before treatment and 3 months after treatment.All patients were followed up for 6 months after treatment.According to whether tumor progression occurred within 6 months after treatment,the 103 patients were divided into a good prognosis group(68 cases)and a poor prognosis group(35 cases).General data were compared between the two groups,and multivariate logistic regression analysis was used to analyze the influencing factors of prognosis in PLC patients.Results The ORR in the combination group was significantly higher than that in the TACE group(P<0.05).At 3 months after treatment,there were no statistically significant differences in ALT,AST,or TBil levels within each group compared to before treatment(P>0.05),and no significant differences were observed between the two groups(P>0.05).At 3 months after treatment,AFP,HGF,and CEA levels in both groups were lower than those before treatment(P<0.05),and the levels in the combination group were lower than those in the TACE group(P<0.05).There were statistically significant differences in tumor diameter,Child-Pugh grade,Barcelona Clinic Liver Cancer(BCLC)stage,presence of portal vein tumor thrombus,prognostic nutritional index(PNI),and treatment regimen between the good prognosis group and the poor prognosis group(P<0.05).Multivariate logistic regression analysis showed that tumor diameter>5 cm(OR=2.707,95%CI:1.424-5.149),Child-Pugh grade B(OR=2.815,95%CI:1.536-5.158),BCLC stage B and C(OR=3.080,95%CI:1.721-5.513),presence of portal vein tumor thrombus(OR=2.726,95%CI:1.560-4.766),and low PNI(OR=2.280,95%CI:1.408-3.692)were influencing factors for prognosis in PLC patients(P<0.05).Conclusion TACE combined with immunotherapy is effective in treating PLC and can significantly inhibit tumor progression.However,prognosis is still influenced by tumor diameter,Child-Pugh grade,BCLC stage,presence of portal vein tumor thrombus,and PNI.Therefore,comprehensive evaluation of PLC patients is necessary to select the optimal treatment regimen.
