Construction of A Risk Prediction Model and Nomogram for Moderate-to-Severe Postoperative Thirst in Elderly Patients Undergoing General Anesthesia Laparoscopic Surgery

Abstract:Objective To analyze the risk factors for moderate-to-severe postoperative thirst in elderly patients undergoing general anesthesia laparoscopic surgery and to construct a nomogram prediction model.Methods A total of 205 elderly patients undergoing general anesthesia laparoscopic surgery admitted to Xiamen University Affiliated Xiang'an Hospital from March 2022 to February 2025 were selected and divided into a moderate-to-severe thirst group(112 cases)and a non-moderate-to-severe thirst group(93 cases)based on whether moderate-to-severe thirst occurred after surgery.Multiple Logistic stepwise regression analysis was used to identify the risk factors for moderate-to-severe postoperative thirst in elderly patients undergoing general anesthesia laparoscopic surgery.A nomogram prediction model for moderate-to-severe postoperative thirst in these patients was constructed based on the risk factors,and the ROC curve was used to evaluate the predictive value.Results Age,diabetes,American Society of Anesthesiologists class Ⅲ-Ⅳ,preoperative fasting time,anesthesia duration,intraoperative fluid replacement type(crystalloid+colloid),intraoperative fluid replacement volume,intraoperative blood loss,intraoperative urine output,operation time,and the proportion of anticholinergic drug usage in the moderate-to-severe thirst group were significantly higher than those in the non-moderate-to-severe thirst group,and the difference was statistically significant(P<0.05).Multifactorial Logistic stepwise regression analysis showed that older age,diabetes,higher intraoperative fluid replacement volume,and use of anticholinergic drugs were risk factors for moderate-to-severe thirst in elderly patients undergoing laparoscopic surgery under general anesthesia(OR=2.377,95%CI:1.600-3.532,P<0.001;OR=2.098,95%CI:1.486-2.962,P<0.001;OR=2.724,95%CI:1.784-4.159,P<0.001;OR=2.625,95%CI:1.732-3.977,P<0.001).The nomogram prediction model for moderate-to-severe postoperative thirst in elderly patients undergoing general anesthesia laparoscopic surgery showed an AUC of 0.905,with a sensitivity of 92.46%and specificity of 68.33%.Conclusion The nomogram model constructed based on risk factors for moderate-to-severe postoperative thirst in elderly patients undergoing general anesthesia laparoscopic surgery demonstrates good predictive performance and has high clinical application value.
