Effects of Tongdu Kaiqiao Electroacupuncture Combined with Repetitive Transcranial Magnetic Stimulation on Neurotransmitters and Rehabilitation in Patients with Post-Stroke Cognitive Impairment

Abstract:Objective To explore the clinical effect of Tongdu Kaiqiao electroacupuncture combined with repetitive transcranial magnetic stimulation(rTMS)in the treatment of patients with post-stroke cognitive impairment(PSCI).Method A total of 126 patients with PSCI admitted to Haian People's Hospital from January 2022 to March 2024 were selected and randomly divided into a control group and an observation group,with 63 cases in each group.On the basis of conventional treatment,the control group received rTMS therapy,while the observation group received additional Tongdu Kaiqiao electroacupuncture treatment for a total of 8 weeks.Levels of neurotransmitters[neuropeptide Y(NPY),gamma-aminobutyric acid(GABA),glucagon-like peptide-1(GLP-1)],cerebral hemodynamics[peak systolic velocity(PSV),peak diastolic velocity(PDV),pulsatility index(PI)],neurocognitive function scores[Montreal Cognitive Assessment(MoCA),Revised Hasegawa Dementia Scale(HDS-R),Stroke-Specific Quality of Life Scale(SS-QOL)],traditional Chinese medicine syndrome scores(dizziness and tinnitus,mental dullness,cognitive decline),and efficacy indicators were observed before treatment and after one course of treatment.Result After one course of treatment,levels of GABA and GLP-1 in the observation group were higher than those in the control group,while NPY was lower than that in the control group(P<0.05).PSV and PI in the observation group were higher than those in the control group,while PDV was lower than that in the control group(P<0.05).MoCA,HDS-R,and SS-QOL scores in the observation group were higher than those in the control group(P<0.05).Scores for dizziness and tinnitus,mental dullness,and cognitive decline in the observation group were lower than those in the control group(P<0.05).The total effective rate in the observation group was higher than that in the control group(P<0.05).Conclusion Tongdu Kaiqiao electroacupuncture combined with rTMS can regulate neurotransmitter metabolism levels,improve cerebral blood flow,promote the recovery of cognitive function,and alleviate clinical symptoms in patients with PSCI.
