Investigation of the Severity of Lumbar Spinal Stenosis Based on Walking Load Test and Nerve Root Sedimentation Sign,and Differences in Nerve Root Sedimentation Sign Classification between Stenotic and Non-stenotic Segments

Abstract:Objective To explore the correlation between the positive rate and classification of nerve root subsidence sign(NRSS)in patients with lumbar spinal stenosis(LSS)and the severity of LSS,and to analyze the difference characteristics of NRSS classification between spinal stenosis and non-stenosis segments.Methods A total of 220 patients diagnosed with LSS in the Fourth People's Hospital of Hengshui City from January 2021 to January 2023 were selected as the research subjects.All patients underwent lumbar MRI examination,and parameters such as the cross-sectional area of the dural sac(DCSA)and were recorded.According to the diagnosis results,the patients were divided into the LSS group(165 cases)and the non-LSS group(55 cases).The LSS group was further divided into the mild LSS group(68 cases),the moderate LSS group(63 cases),and the severe LSS group(34 cases)according to the Schizas classification based on the DCSA.The Barz standard was used to classify NRSS-positive patients into three types:a,b and c.All patients underwent walking load tests and visual visual analogue scale(VAS)score evaluations.Differences in the positive rate and classification of NRSS among the groups,as well as the correlation between NRSS classification and LSS severity,were analyzed.Results The positive rate of NRSS in the severe LSS group(94.12%)>in the moderate LSS group(65.08%)>in the mild LSS group(36.76%)>in the non-LSS group(23.64%)(P<0.05).The NRSS classification was negatively correlated with the smallest DCSA value(r=-0.685,P<0.001),negatively correlated with the walking distance(r=-0.742,P<0.001),and positively correlated with the VAS score(r=0.657 and r=0.721,both P<0.001).There were statistically significant differences in the smallest DCSA value,walking distance and VAS score among different NRSS classification groups(P<0.05).Among them,patients with type c had the smallest DCSA value,the shortest walking distance and the highest VAS score.Conclusion The positive rate and classification of NRSS are closely related to the severity of LSS.Type c is predominantly found in patients with severe LSS and can be used as an important imaging indicator for evaluating the severity of LSS.The NRSS classification of spinal stenosis segments and non-stenosis segments is significantly different,providing a new imaging basis for the clinical diagnosis and treatment of LSS.
