Diagnostic Value of Musculoskeletal Ultrasound Combined with Serum OCN and PICP Levels in Knee Osteoarthritis

Abstract:Objective To investigate the diagnostic value of musculoskeletal ultrasound(MSUS)combined with serum osteocalcin(OCN)and procollagen typeⅠC-terminal propeptide(PICP)levels in knee osteoarthritis(KOA).Methods A total of 200 patients with highly suspected KOA admitted to Daqing Longnan Hospital were prospectively selected from January 2024 to December 2024.All patients underwent MSUS examination,and their OCN and PICP levels were measured.According to magnetic resonance imaging results,the patients were divided into a KOA group(154 cases)and a non-KOA group(46 cases).Baseline data,MSUS parameters[synovial thickness(ST),suprapatellar effusion(SPE),femoral condylar cartilage thickness(FCCT),osteophyte height(OPH)],OCN,and PICP were compared between the two groups.The diagnostic efficacy of MSUS parameters,OCN,PICP,and their combination for KOA was evaluated using ROC curve analysis.Patients in the KOA group were further classified into grade 2(103 cases),grade 3(38 cases),and grade 4(13 cases)according to the Kellgren-Lawrence(K-L)grading system.Differences in MSUS parameters,OCN,and PICP were analyzed among KOA patients with different K-L grades.Results Compared with the non-KOA group,the KOA group had higher levels of ST,SPE,OPH,OCN,and PICP,and a lower level of FCCT(P<0.01).ROC curve analysis showed that the area under the curve for the combined diagnosis of KOA was 0.960.The area under the curve for ST,SPE,FCCT,OPH,OCN,and PICP alone were 0.854,0.930,0.667,0.944,0.741,and 0.882,respectively.The combined detection was significantly better than ST,FCCT,OCN,or PICP alone(P<0.01).Patients with grade 4 and grade 3 KOA had higher levels of ST,SPE,OPH,OCN,and PICP and lower levels of FCCT than those with grade 2(P<0.05).Grade 4 patients had higher levels of ST,SPE,OPH,OCN,and PICP and lower levels of FCCT than grade 3 patients(P<0.05).Conclusion MSUS combined with OCN and PICP has high diagnostic value for KOA and is closely related to the K-L grading of patients.
