Effect of Continuous Irrigation Combined with VSD on Wound Repair and Serum CRP,IL-6 and ESR Levels in Patients with Open Fracture

Abstract:Objective To investigate the effect of continuous irrigation combined with vacuum sealing drainage(VSD)on wound repair and infection rate in patients with open fracture.Methods Eighty patients with open fractures treated in the Orthopedics Department of the First Affiliated Hospital of Nanjing Medical University from November 2021 to February 2024 were selected and divided into a control group(40 cases)and an observation group(40 cases)according to the random number table method.After thorough debridement,the control group received simple VSD treatment,while the observation group received continuous irrigation in addition to VSD.Clinical efficacy was evaluated at the 3-month follow-up.Wound repair(complete wound healing time and Vancouver Scar Scale score),infection occurrence during follow-up(infection rate,time of infection onset,and infection duration),and inflammatory levels(C-reactive protein,interleukin-6,erythrocyte sedimentation rate)were compared between the two groups.Other adverse reactions were also recorded.Results The total effective rate of the observation group was higher than that of the control group(95.00%vs 77.50%,P<0.05).The observation group had a shorter complete wound healing time and a lower Vancouver Scar Scale score than the control group[(67.84±8.89)d vs(82.96±11.64)d,P<0.05;(5.21±1.56)points vs(7.23±2.51)points,P<0.05].The infection rate,time of infection onset,and infection duration in the observation group were lower than those in the control group[7.50%vs 25.00%,P<0.05;(9.16±2.81)d vs(12.15±3.47)d,P<0.05;(12.61±2.89)d vs(18.42±3.57)d,P<0.05].Levels of C-reactive protein,interleukin-6,and erythrocyte sedimentation rate decreased over time in both groups after treatment,and were lower in the observation group at each time point(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions such as fever,wound bleeding,drainage tube blockage,scar hyperplasia,and sub-membrane fluid accumulation between the two groups(P>0.05).Conclusion Continuous irrigation combined with VSD treatment based on VSD therapy is effective for open fractures.It can significantly shorten wound repair time,improve wound repair quality,and reduce the incidence of wound infection.
