Diagnostic Value of HPV Testing Combined with DNA Ploidy Analysis for Cervical Intraepithelial Neoplasia

Abstract:Objective To explore the diagnostic value of HPV testing combined with DNA ploidy analysis in cervical intraepithelial neoplasia(CIN).Methods A total of 55 patients suspected of high-risk HPV infection who were examined at the Affiliated Stomatology Hospital of Jinan University between January 2023 and June 2023 were selected and underwent DNA ploidy analysis.Using pathological diagnosis from colposcopy-guided cervical biopsy as the gold standard,the sensitivity,specificity,and overall accuracy of HPV testing combined with DNA ploidy analysis for≥CIN Ⅰand≥CIN Ⅱ were calculated.Results The positive rate of high-risk HPV among the 55 subjects was 90.91%(50/55).Pathological results after cervical biopsy confirmed 8 cases of cervicitis,17 cases of CIN Ⅰ,16 cases of CIN Ⅱ,10 cases of CIN Ⅲ,and 4 cases of cervical cancer.DNA ploidy analysis detected 40 positive cases(72.73%),8 suspicious cases(14.55%),and 7 negative cases(12.73%).Using pathological results≥CIN Ⅰ as the clinical diagnostic target(i.e.,considering CIN Ⅰ,CIN Ⅱ,CIN Ⅲ,and cervical cancer as positive),the sensitivity,specificity,and overall accuracy of HPV testing were 97.87%(46/47),50.00%(4/8),and 90.91%(50/55),respectively,showing moderate consistency with pathological diagnosis(Kappa=0.567).The corresponding values for DNA ploidy analysis were 85.11%(40/47),100.00%(8/8),and 87.27%(48/55),indicating good consistency(Kappa=0.624).The combination of HPV testing and DNA ploidy analysis yielded sensitivity,specificity,and overall accuracy of 100.00%(47/47),75.00%(6/8),and 96.36%(53/55),respectively,demonstrating excellent consistency with pathological diagnosis(Kappa=0.837).When using pathological results≥CIN Ⅱ as the clinical diagnostic target(i.e.,considering CIN Ⅱ,CIN Ⅲ,and cervical cancer as positive),the sensitivity,specificity,and overall accuracy of HPV testing were 100.00%(30/30),20.00%(5/25),and 63.64%(35/55),respectively,showing fair consistency with pathological diagnosis(Kappa=0.214).For DNA ploidy analysis,the values were 93.33%(28/30),52.00%(13/25),and 74.55%(41/55),indicating moderate consistency(Kappa=0.469).The combination strategy showed sensitivity,specificity,and overall accuracy of 100.00%(30/30),56.00%(14/25),and 80.00%(44/55),respectively,with moderate consistency(Kappa=0.581).Conclusion High-risk HPV detection combined with DNA ploidy analysis has certain value in diagnosing CIN,especially for the diagnosis of CIN I and above lesions with high total coincidence rate,and it can be used as a mainstream means of CIN screening for high-risk HPV subjects.
