A Comparative Study of Cold Snare Polypectomy and Loop High-Frequency Electrocautery in the Treatment of Small-Diameter Flat Sessile Colorectal Polyps

Abstract:Objective To analyze and compare the effects of cold snare polypectomy(CSP)and loop high-frequency electrocautery on patients with small-diameter flat sessile colorectal polyps.Methods A total of 80 patients with small-diameter flat sessile colorectal polyps treated with CSP at Dezhou Hospital of Shandong University Qilu Hospital from October 2024 to February 2025 were included in the cold resection group.Another 80 patients with small-diameter flat sessile colorectal polyps treated with loop high-frequency electrocautery during the same period,matched by sex and age,were selected as the electrocautery group.The following outcomes were compared between the two groups:surgery-related indicators(colonoscopy procedure time,complete polyp resection rate),postoperative recovery time(time to first flatus,time to ambulation),total incidence of complications(delayed bleeding,infection,immediate perforation,delayed perforation),total treatment cost,and pain level[visual analog scale(VAS)]at 2,8,and 12 hours after surgery.Results Compared with the electrocautery group,the cold resection group had significantly shorter colonoscopy procedure time and time to first flatus(P<0.05),lower total treatment cost(P<0.05),and a lower VAS score at 2 hours after surgery(P<0.05).There were no significant differences between the two groups in the complete polyp resection rate,time to ambulation,total incidence of complications,or VAS scores at 8 and 12 hours after surgery(P>0.05).Conclusion Both CSP and loop high-frequency electrocautery are equally effective in treating patients with small-diameter flat sessile colorectal polyps,with a low risk of complications.However,CSP is associated with shorter colonoscopy procedure time,less pain shortly after surgery,and lower treatment cost.
