Analysis of Influencing Factors for Secondary Subjective Tinnitus in Patients with Secretory Otitis Media

Abstract:Objective To explore the occurrence and influencing factors of secondary subjective tinnitus in patients with secretory otitis media,so as to provide a theoretical basis for clinical prevention and treatment.Methods A total of 217 patients with secretory otitis media admitled to the First Affiliated Hospital of Air Force Medical University from June 2022 to June 2024 were selected as the study subjects and divided into a tinnitus group(73 cases)and a non-tinnitus group(144 cases)based on the occurrence of secondary subjective tinnitus.Clinical data were collected,and univariate analysis,correlation analysis,and multivariate logistic regression analysis were used to identify independent influencing factors.The predictive value of key indicators was evaluated using receiver operating characteristic(ROC)curve analysis.Results The tinnitus group had longer disease duration,higher eustachian tube dysfunction questionnaire-7(ETDQ-7)scores,higher white blood cell(WBC)count,higher neutrophil(NEUT)count,lower lymphocyte(LYM)count,and higher interleukin-6(IL-6)levels,as well as higher proportions of allergic rhinitis,severe hearing impairment,habitual nocturnal wakefulness,and no previous treatment history compared to the non-tinnitus group(P<0.05).Disease duration,ETDQ-7 score,WBC,NEUT,allergic rhinitis,and staying up late were positively correlated with tinnitus occurrence,while previous treatment history and LYM were negatively correlated(P<0.05).Disease duration,ETDQ-7 score,WBC,NEUT,IL-6,and previous treatment history were identified as independent influencing factors for tinnitus occurrence.ROC curve analysis showed that the AUC values for WBC,disease duration,previous treatment history,ETDQ-7 score,NEUT,and IL-6 were 0.672,0.926,0.690,0.665,0.725,and 0.858,respectively,all indicating certain predictive value for tinnitus occurrence.The optimal cut-off values for WBC,disease duration,ETDQ-7 score,NEUT,and IL-6 were 7.35×109/L,5.85 months,10.50 points,59.82×109/L,and 4.35 ng/L,respectively.Conclusion The occurrence of secondary subjective tinnitus in patients with secretory otitis media is influenced by multiple factors such as disease duration,eustachian tube function,inflammatory response,and previous treatment history.Clinical attention should be given to patients with long disease course,eustachian tube dysfunction,and elevated inflammatory markers for early intervention to reduce the risk of tinnitus.
