The Relationship between Neutrophil to Apolipoprotein A1 Ratio and Short-Term Prognosis in Patients with Acute Cerebral Infarction

Abstract:Objective To explore the relationship between neutrophil/apolipoprotein A1 ratio(NAR)and short-term prognosis in patients with acute cerebral infarction.Methods A total of 172 patients with acute cerebral infarction admitted to Yuancheng District People's Hospital of Heyuan City from January 2023 to January 2025 were selected as the study group,and 60 healthy individuals during the same period were selected as the control group.According to the modified Rankin Scale(mRS)score at 90 days after discharge,the study group was divided into a good prognosis group(n=114)and a poor prognosis group(n=58).The levels of neutrophils(NEU)and apolipoprotein A1(ApoA1)were measured,and NAR was calculated.The predictive value was analyzed using the receiver operating characteristic(ROC)curve,and influencing factors were analyzed by multivariate stepwise Logistic regression.Results The NEU and NAR in the study group were higher than those in the control group,while ApoA1 was lower(P<0.05).The NEU and NAR in the poor prognosis group were significantly higher than those in the good prognosis group,and ApoA1 was significantly lower(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of NAR for predicting short-term prognosis in patients with acute cerebral infarction was 0.902,which was significantly higher than that of NEU(0.778)and ApoA1(0.824)alone(Z=11.693,9.701,P<0.001).Age,proportion of atrial fibrillation history,NIHSS score on admission,C-reactive protein,and D-dimer in the poor prognosis group were higher than those in the good prognosis group,and the time from onset to admission was longer(P<0.05).Multivariate stepwise Logistic regression analysis showed that advanced age,high NIHSS score on admission,high C-reactive protein,and NAR≥5.09 were independent risk factors for short-term prognosis in patients with acute cerebral infarction(P<0.05).Conclusion An elevated NAR is associated with poor short-term prognosis in patients with acute cerebral infarction,and NAR has good predictive value for the short-term prognosis of these patients.
