The Relationship between the Serum SAA/CRP and Disease Prognosis in Patients with COPD Complicated by Pneumonia

Abstract:Objective To explore the relationship between the serum amyloid A/C-reactive protein ratio(SAA/CRP)and disease prognosis in patients with chronic obstructive pulmonary disease(COPD)complicated by pneumonia.Methods A total of 120 patients with COPD complicated by pneumonia who were admitted to Yixing People's Hospital from January 2023 to January 2025 were selected.According to prognosis evaluation results,they were divided into a poor prognosis group(29 cases)and a good prognosis group(91 cases).The value of SAA,CRP,and the SAA/CRP in predicting disease prognosis in COPD patients with pneumonia was assessed using ROC curves,and multivariate Logistic stepwise regression analysis was used to identify factors influencing disease prognosis.Results The levels of SAA,CRP,and the SAA/CRP were higher in the poor prognosis group than in the good prognosis group(P<0.05).The AUC of the SAA/CRP for predicting disease outcome in COPD patients with pneumonia was 0.907,significantly higher than that of SAA(0.851)and CRP(0.762)alone(P<0.01).The poor prognosis group had higher age,longer COPD duration,higher clinical pulmonary infection score,procalcitonin level,white blood cell count,neutrophil count,proportion of patients with grade Ⅲ-Ⅳ airflow limitation,and rate of invasive procedures compared to the good prognosis group(P<0.05).Multivariate Logistic stepwise regression analysis showed that prolonged COPD duration,invasive procedures,elevated clinical pulmonary infection score,SAA≥199.66 mg/L,and SAA/CRP≥3.45 were independent risk factors for disease outcome in COPD patients with pneumonia(OR=1.887,95%CI:1.238-2.876,P<0.05;OR=2.425,95%CI:1.542-3.814,P<0.05;OR=2.081,95%CI:1.331-3.254,P<0.05;OR=2.751,95%CI:1.348-5.615,P<0.05;OR=3.096,95%CI:1.565-6.123,P<0.05).Conclusion An elevated SAA/CRP is closely associated with disease prognosis in patients with COPD complicated by pneumonia and has high predictive value,making it a potential biomarker for evaluating disease prognosis in these patients.
