Effects of Postauricular Subperiosteal Injection Versus Systemic Glucocorticoid Therapy on Hearing in Patients with Low-Frequency Descending Sudden Sensorineural Hearing Loss

Abstract:Objective To investigate the clinical effects of postauricular subperiosteal glucocorticoid therapy versus systemic glucocorticoid therapy in patients with low-frequency sudden sensorineural hearing loss.Methods A total of 102 patients with low-frequency sudden sensorineural hearing loss admitted to Dazhou Central Hospital were prospectively selected from May 2023 to February 2025,and were divided into a control group(51 cases)and an observation group(51 cases)using the random number table method.The control group received systemic glucocorticoid therapy,while the observation group received postauricular subperiosteal glucocorticoid therapy.The two groups were compared regarding clinical efficacy,symptom scores,low-frequency hearing threshold changes,blood biochemical indicators,and adverse reactions.Results The total clinical effective rate was higher in the observation group than in the control group(96.08%vs 84.31%),and the difference was statistically significant(P<0.05).After treatment,scores for ear fullness,low-key tinnitus,and vertigo symptoms,as well as 250 Hz and 500 Hz hearing thresholds,were lower than before treatment in both groups,and were lower in the observation group than in the control group,with statistical significance(P<0.05).Levels of fasting plasma glucose,alanine aminotransferase,and serum creatinine increased compared to before treatment in both groups,but were lower in the observation group than in the control group,with statistical significance(P<0.05).The incidence of adverse reactions was lower in the observation group than in the control group(5.88%vs 19.61%),and the difference was statistically significant(P<0.05).Conclusion Compared with systemic glucocorticoid therapy,postauricular subperiosteal glucocorticoid therapy provides superior clinical efficacy for patients with low-frequency sudden sensorineural hearing loss,effectively alleviates symptoms,significantly improves low-frequency hearing thresholds,and results in fewer adverse reactions.
